About SLG
Founded and Headquartered in Danielson in 2006, with additional program locations established in Avon and Bethany Connecticut, The Supported Living Group (SLG) provides industry leading non-medical community based and person focused support services to individual's utilizing Acquired Brain Injury (ABI) Waiver I & II, Department of Developmental Services (DDS) Home and Community Waiver, Connecticut Home Care Program for Elders (CHCPE), and Personal Care Assistant (PCA) Waiver supports across Connecticut.

Employees are required to have the following:
- Desire to improve the quality of life for a person with a Brain Injury
- Experience working with disabled individuals preferred
- A high school education (or GED)
- Valid Drivers license and reliable transportation
- Must be at least 18 years or older with personal cell phone

Work Schedule
This job will require you to work in the homes and communities of the individuals we serve. While each individual may have different hours, we have 1st, 2nd and 3rd shift responsibilities, 7 days a week. The majority of the individuals that we serve have services that require someone to be in their home 24 hours a day - perfect for people who need a flexible schedule.
Services you will be providing:
Companion (Comp): Services are provided in accordance with therapeutic goals. These services include supervision, socialization, assistance and/or supervision of meal preparation, housekeeping tasks that are supplementary to the ability of the individual, and other needs.
**Providers must complete a Supplemental application and attend the ACR ABI informational session.
Personal Care Assistant (PCA): Provides assistance with the following: eating, bathing, dressing, personal hygiene, and other activities of daily living; or supervision and cueing of these activities without actual hands-on assistance.
ABI Recovery Assistant I & II (RA): Supports to promote the individual's abilities to maintain and foster community living skills, in accordance with therapeutic goals outlined in the individual's service plan. Services may include improvement of socialization, self-advocacy, and the development of natural supports. Supports are to promote safety through monitoring, assist with activities of daily living (both hands-on and cueing), medication compliance, and community integration.
**Providers must complete an RA application and attend the 1-day ACR sponsored RA training**
Vocational (Pre-Voc/SE): Services provide volunteer or paid learning and work experience, where the individual can develop general job and non-job-task-specific strengths and skills that contribute to employment or employability in an integrated work setting. Services are intended to develop and teach general skills, such as the ability to communicate effectively; comply with generally accepted community and/or workplace conduct and dress; follow directions; attend to tasks; develop strategies to solve problems; and comply with general safety and mobility standards. Services shall be provided in the individual's home or in an integrated work setting, based on the individual's needs and preferences. The individual shall have employment-related goals in the person-centered service plan.
Independant Living Skills Training (ILST): SLG's Independent Living Skills Training Services are individually tailored to individual life goals and recovery targets. SLG's credentialed ILST level staff educate, supervise, and assist our ABI Waiver Participants to develop and maintain skills in areas such as self-care, medication management, task completion, communication, interpersonal interactions, socialization, sensory/motor ability, community transportation competence, behavioral outburst reduction, prproblem-solvingmoney management, and household management.
