Responsibilities:
- Provide personal care to patients in their homes, including bathing, dressing, and grooming.
- Assist with mobility and transfers, using proper lifting techniques.
- Monitor and record vital signs, such as blood pressure and temperature.
- Assist with meal planning and preparation, ensuring dietary needs are met.
- Document medical information accurately and maintain patient records.
- Provide emotional support and companionship to patients.
- Follow care plans and communicate any changes in the patient's condition to the appropriate healthcare professionals.
Qualifications:
- High school diploma or equivalent.
- Certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) preferred.
- Experience working with individuals with developmental disabilities is a plus.
- Knowledge of medical documentation and record keeping.
- Ability to lift and assist patients with mobility needs.
- Proficiency in cooking nutritious meals for patients with dietary restrictions.
- Familiarity with catheter care, tube feeding, and hospice care procedures is desirable.
- Excellent communication skills and the ability to work well with a team.
