Position Purpose:
Our hospice/Home Health aides provide paraprofessional services to our patients in their homes, following physician orders and in compliance with the state's Nurse Practice Act, any applicable licensure/certification requirements and our policies and procedures.
What You Will Do:
- Follows care plan developed by RN.
- Reports client changes in condition to RN, as described on care plan.
- Assists clients with personal care activities including bathing, mouth care, denture care, skin care, grooming, bathroom privileges, transferring, ambulation, and dressing and undressing activities.
- Assists clients with household tasks including, planning and preparing meals, feeding, light housekeeping, changing linens, making bed, and laundering clothes and bedding.
- Measures and records client's oral, rectal and axillary temperatures, pulse respiration and blood pressure as ordered.
- Performs range of motion and other simple procedures as an extension of therapy services as assigned.
- Assists clients in self-administration of medication. This procedure must be assigned by an RN and documented in the clinical record. The Home Health / Hospice Aide shall not administer medication of any kind
Minimum Qualifications:
- High school diploma or GED required.
- Completion of federally compliant training program as required by law/regulation and successful completion of a competency evaluation program.
- Documentation of one year's employment as an aide in an institution or home care setting.
- Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with agency's client and their family.
- Must accept responsibility for maintaining skills, learning and adhering to agency's policies and procedures on an on-going basis.
- Must have current driver's license and reliable transportation to and from work site.
