The Holyoke Visiting Nurses Association, Inc. is looking to hire a Home Health Aide working full time for home care & hospice experience.
- Practice proper infection control and use standard precautions.
- Perform minimal light housekeeping tasks per the instruction on the HCA care plan if time allows.
- Assist or provide personal hygiene (sponge bath, shower or tub bath, bed bath, and mouth care).
- Assist clients with dressing.
- Remind clients to take medication(s).
- Plan and prepare or assist in preparing nutritious meals or prescribed diets.
- Assist the patient to perform prescribed physical therapy, speech therapy and occupational therapy exercises as instructed by the appropriate professional.
- Assist in the use, care and cleaning of prescribed medical equipment (hospital bed, cane, walker, quad cane, etc.).
- Use a Hoyer lift or similar equipment to transfer patient to and from the bed, toilet, chair, tub, etc.
- Manually assist disabled or handicapped patients with transfers.
- Observe and accurately report all changes in the patient’s physical, emotional or social condition on a timely basis.
- Identify and report unsafe and/or potentially unsafe practice in patient care to supervisor and to nurse or therapist.
- Maintain confidentiality during and after working hours.Follow assigned schedules and notify office of changes.
- Accurately complete required work time and charting reports and return them to the office on a timely basis.
- Provide care and emotional support to persons who are acutely or chronically ill, which may include involvement in sensitive family relationships, in a supportive, non-judgmental, non-discriminatory manner.
- Complete job responsibilities in a professional, positive manner in accordance with Agency’s philosophy, goals and policies.
- Attend scheduled in-service programs (twelve hours annually).
- Perform other duties as responsibility necessitates or as required.
