Job Summary:
The home care assistant (CNA/HHA) will assist clients, in their place of residence, with personal healthcare and health maintenance activities as assigned. Includes, but is not limited to: transferring, health maintenance activities, feeding, mobility, and exercises.
Job Responsibilities:
· Assist clients with tasks within the scope of a Home Health Aide based on the Plan of Care (POC).
· Communicate effectively with administrative staff regarding assessments of the client/family needs, including reporting any significant changes in client's condition.
· Provide care as outlined in the Home Health Aide Care Plan and accurately document care provided.
· Contribute to continuity of care and effective communications between all members of the care team, and regular participation in Case conferences/Team Meetings.
· Demonstrate commitment and professional growth by participating in in-service programs and maintaining/improving competency. Completes training or in-services in a timely manner as assigned by your supervisor.
· Meet mandatory continuing education requirements of the Agency/licensing board.
Experience:
· 1- year experience as a Home Health Aide or certified nursing assistant in a Home Health setting. Successful completion of formal training in nursing arts may be substituted for the qualifying experience, subject to applicable State law, regulation, or rules.
· Preferred, Home Health experience.
Skills:
· Ability to establish and maintain effective working relationships with all segments of the staff, and the Agency leadership team.
· Shall be able to read, write and comprehend English.

CNA/HHA Requirements:
· A current Certificate that is in good standing to practice as an CNA/HHA in any/all applicable states.
· Home Health Aide Certificate or other similar certification held in good standing. Certification requirements may vary by state and/or participating program.
· Formal certificate of completion for an HHA/CNA training program that meets or exceeds standards established by Health and Human Services. Completion of all in-service hours per year. Ongoing requirements may vary by state and/or participating program.
· Current CPR certification.
· A valid driver’s license and reliable transportation, insurance required.
· Reliable transportation and auto liability insurance.
· Active listening skills and empathy.
· Physical and mental strength.
· skills as defined as generally accepted standards of practice.
· The ability to follow instructions but also to act independently if necessary.
Education:
· High School Graduate or equivalent
