About the Organization and Position
Well known Non-Profit Health Agency is hiring for a Home Care Case Manager.
This wonderful non-profit serves people age 60 and older, adults living with disabilities, and their families and caregivers in Lynn, Lynnfield, Nahant, Saugus, and Swampscott, Massachusetts for more than 45 years. They provide a continuum of community health and social services to help people maintain their independence, safely and with dignity. Providing a broad range of services including Home Care services; nutrition programs; transportation assistance; housing supports; clinical and protective services etc. The agency recently received a second prestigious 3-year accreditation for excellence in case management and care coordination from the National Committee for Quality Assurance (NCQA).
The Home Care Case Manager (CM) has daily responsibility for developing comprehensive person-centered service plans and authorizing, coordinating, and monitoring community-based services to an assigned number of consumers. The process begins with a comprehensive needs assessment, completed as part of an
interdisciplinary team.

Responsibilities
Visits consumers at least twice yearly in their home, or more frequently as determined by
program-specific requirementsConducts a comprehensive assessment of consumers’ needs using required EOEA tools and
determines eligibility for service.In collaboration with the consumer and/or their caregiver(s), develops an appropriate person-
centered service plan to meet consumer needs working within resource capacities and guidelinesCollaborates and coordinates needed services with other agencies on behalf of the consumers
Ensures that care plans incorporate the preferences and values of the consumer and are developed
in conjunction with the consumer and caregiver using a person-centered planning approachProvides crisis case management as needed
Reassesses the consumer’s needs and adjusts services as required
Conducts Initial Assessment for new referrals as needed
Experience
- Minimum of a Bachelor’s Degree in Human Services or 50% of Bachelor’s Degree
completed with degree obtained within three (3) years of employment required - Minimum of one (1) to two (2) years’ experience in Human Services
- Must demonstrate a passion for supporting consumers in achieving their personal
goals related to wellness and community living
