Position Responsibilities: Assist Care Managers with administrative and consumer related functions to ensure that all data entry and casework processing is accurate and timely. The Case Aide handles the opening and closing of cases and entering consumer information in A&D.
Essential Functions: (Core duties or tasks that are fundamental and not marginal to the performance of the job)
Supports Intake processing by preparing consumer charts, creating care plans, and entering financial information into A&D.
Completes program transfers for existing consumers in A&D.
Processes the closing of consumer records.
Observe all confidentiality regulations and protect consumer information.
Complete documentation in consumer database and abide by regulations as set forth by agency and EOEA.
Attend mandatory meetings and trainings including Team Meeting, General Staff Meeting and Home Care Staff Meeting.
Other duties as assigned by Home Care Management Staff.
Qualifications: Associates Degree and 1 year relevant work preferred. High School diploma with significant relevant work experience may be substituted for degree. Work in a human service environment a plus. Strong computer skills including proficiency in Microsoft Office applications are required. Must have outstanding time management skills and be able to handle multiple tasks at a time. Must be responsible and have compassion for older people. Excellent interpersonal and communication skills a must in this team-oriented position.
Hours: 37.5 per week
