Position Overview:

We are looking for motivated and compassionate Home-Based Case Workers to join our team. In this remote role, you will have the opportunity to manage your own caseload, set your schedule, and work in the community.   • Document client Progress Notes and Reports in Case Management Pro (CMP)

Responsibilities:

  • Conduct comprehensive assessments of clients' needs and develop individualized care plans.
  • Collaborate with other healthcare professionals and community resources to coordinate services for clients.
  • Maintain accurate and up-to-date client records and case notes.
  • Monitor client progress and make adjustments to care plans as needed.
  • Advocate for clients' rights and access to necessary services.
  • Participate in occasional team meetings and case reviews.
  • Stay informed about changes in relevant laws, regulations, and best practices in case management.
  • Maintain client confidentiality and adhere to ethical standards.

Qualifications:

  • Associate’s or Bachelor's degree in social work, psychology, or a related field
  • Experience in case management or a related field appreciated but not required
  • Excellent communication and interpersonal skills.
  • Strong organizational and time management abilities.
  • Ability to work independently and effectively in a remote environment.
  • Empathy, compassion, and a genuine desire to help others.
  • Proficiency in using remote communication and case management tools.
  • Must be at least 21 years of age
  • A valid driver's license and insurance with no more than 6 points on driving record
  • Travel between appointments to service clients within their community
  • Knowledgeable regarding social service systems within the state
  • Knowledge of and adherence to current confidentiality practices
  • Appreciation for and ability to respond to cultural differences
  • Develop relevant networks of contacts
