The Home Based Acute Care Registered Nurse (RN) provides professional, comprehensive nursing care for Acute Care patients in the patient's home environment in collaboration with the HBAC Command Center. Accountable for the delivery of coordinated, safe, compassionate, therapeutic, evidence-based quality care to patients and families, based on individual physical, emotional, and spiritual needs, and appropriate care strategies throughout the lifespan.

Required qualifications:

  • Education sufficient for licensure
  • Upon hire: Washington Registered Nurse License Or
  • Upon hire: National Provider BLS - American Heart Association
  • Upon hire: In compliance with the Providence Health & Services, criminal background check is required for this position, based on assigned duties
  • Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request. See policy for additional information.
  • 1 year of Medical/surgical experience in an Acute Care setting

Preferred qualifications:

  • Bachelor's Degree in Nursing
  • IV/Infusion skills
  • Recent Home Health experience
  • Point of Care experience

About the Team

Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.

HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses. Our core competencies include restorative care, longitudinal care and care through the end of life. Our service lines include assisted living/skilled nursing/rehabilitation, home health, home infusion/pharmacy services, home medical equipment, hospice and palliative care, Program of All-Inclusive Care for the Elderly (PACE), personal home services/private duty care and supportive housing.
