Position Description:

The Home Asthma Specialist (HAS) will serve as a liaison between families whose children are diagnosed with asthma, the New York-Presbyterian Health system, and Public Health Solutions’ network of community resources. The HAS will perform home visits for enrolled patients to complete home environmental trigger assessment, SDOH screening, and provide Asthma Self-Management Education and referrals to other home-based services such as integrated pest control management. During the home visits, the HAS will also facilitate telehealth medical visits for children with asthma and a Pediatric Asthma Provider (Nurse Practitioner or MD) using technology and equipment provided by the program. They will empower patients and their caregivers to improve health outcomes by providing peer-based, culturally sensitive support and health education. The HAS will also make connections to primary care and community-based social services that support families’ health and well-being and reduce stress.

Participating families will receive comprehensive information from their CHW regarding: 1) understanding their child's condition, 2) knowing how to access health care for their child, and 3) learning about self-management and prevention strategies to keep their child's condition under control. The HAS will also provide environmental trigger control products as needed and help families to connect to services for issues such as food insecurity, immigration, housing, education, and employment.

Specifically, the Home Asthma Specialist will:

  • Manage designated caseload and meet all stated program goals.
  • Conduct up to 4 home visits per family to provide in-person asthma education, support and referrals to services. Must be willing to travel throughout NYC boroughs and to Nassau County if needed. Travel costs will be covered.
  • Conduct home environmental assessments and coordinate with the NYP clinical team to ameliorate possible home environmental triggers of asthma.
  • Collaborate with the healthcare team at NYP and with Public Health Solutions’ staff in the maternal child health unit and more.
  • Complete appropriate documentation of assessments and educational sessions at program intervals.
  • Participate in training and staff meetings necessary to perform the tasks, and achieve the goals, of the Pediatric Asthma program.
  • Participate in NYP Asthma Program team meetings at least monthly to review cases and evaluate program progress.
  • Other tasks as assigned.

Qualifications and Requirements:

  • Interest in maternal child health, health justice, and community-based support for families.
  • Bilingual preferred: English, and another language frequently spoken by NYP patients. Spanish preferred.
  • 2 years’ Community Health Worker or comparable experience preferred.
  • Self-organized, and ability to work well independently and as part of a team.
  • Salary: $27.4726 - $27.4726
