HIV Testing Coordinator
Full Time, 36 hours per week

Job Overview:
The HIV Testing Coordinator provides in-depth information, clarification and interpretation of internal and community-based HIV testing to promote education, awareness, and enhance treatment options with the goal of HIV eradication. The HIV Testing Coordinator will ensure program compliance with institutional rules, regulations, policies and procedures. This position will build relationships organizational-wide with the implementation of the HIV program. Through these relationships the HIV Testing Coordinator will serve as a liaison or link between patient and resources to facilities access to care, increase education on HIV resources, improve quality of care, and address the effects of social determinants of health on existing care for patients.
Duties:

  • Provides individual support to patients working toward self-management of health conditions, primarily in a home or community setting.
  • Contributes to the development of the patient based on the identified patient problems/needs.
  • Provide referrals and follow-up on referrals.
  • Coordinate the addition and expansion of walk-in, opt-out, at-home, and community-based HIV testing.
  • Work with the HIV grant-funded team to address testing needs for PrEP initiation and maintenance.
  • Serve as a resource for providers and staff, providing sensitivity training and other education pertaining to HIV and HIV testing, as needed.
  • Assist in the preparation of internal and grant reporting on HIV testing and linkage to treatment.
  • Serve as an advocate for HIV testing, prevention, and linkage to treatment and the de-stigmatization to HIV and HIV testing.
  • Will work with APO for coordination of education, processes, and inventory.
  • Maintain and coordinate communication among patient, staff, and physician.
  • Works in a manner that builds individual and community capacity.
  • Provides basic HIV testing using Chembio and Alere.
  • Provides culturally appropriate health education and information to advance patient's health literacy related to their conditions.
  • Follow up on appointments or instructions from the healthcare team by making phone or in-clinic communication.

Requirements
Required Qualifications:
  • High school diploma or GED.
  • Valid driver's license.

Preferred Qualifications:
  • 1-3 years relevant clinical experience or current valid medical assistant credentials.
