Eligibility in 2 installments: $2,500 at 90 days of employment / $2,500 at 180 days of employment
Starting Pay Rate: $45 - $50 / hour

Primary Responsibilities:
The Community Health Nurse will contribute to the assessment of residents, implement health care services and collaborate with the Community Health Director to provide health promotion and monitoring of residents.
Knowledge, Skills, and Abilities:
- Ability to provide resident and family education
- Ability to schedule tasks effectively to meet strict deadlines and within allotted time
- Ability to maintain resident confidentiality to assure all rights are protected
- Ability to read, write and communicate in English; Excellent interpersonal skills
- Computer proficiency

Requirements:
- Proof of valid RN license in good standing from the State Of Washington
- Must be authorized to work in the US
- Minimum 2 years’ experience working with older adults; nursing experience in an assisted living facility, preferred
- Must be able to complete Modified Fundamentals of Caregiving
- Must complete Caregiver Mental Health and Dementia specialty training
- Must have current CPR certificate

- A DSHS background check may be conducted after a conditional offer of employment
