REPORTS TO: YEVP Program Manager

CLASSIFICATION: Full-Time, 1.0 FTE

FLSA STATUS: Non-Exempt

LOCATION: Clackamas,OR

COMPENSATION: $23.07 Hourly

CANDIDATE PROFILE

Latino Network seeks a bilingual, mission driven and community service mindset individual with bi-cultural proficiency to provide case management support, coverage and support to community members who are justice involved. The ideal candidate will have experience working with families in the target population and have excellent communication and strong interpersonal skills.


POSITION DESCRIPTION 

The HEART Family Navigator will play an integral role in navigating youth and their families to resources, facilitating and/or co-facilitate teaching circles, strengthen cultural values/identity, advocate/ support family needs through various institution such as but not limited to juvenile system, courts, financial stability support, and schools. This partnership is between Latino Network and Clackamas County Juvenile Department. This program aims to decrease recidivism and increase visibility in the youth and families of Clackamas County that are justice involved, by responding to youths and family needs.

The Family Navigator will work alongside Clackamas County juvenile staff, stakeholders and community partnerships. Latino Network HEART Family Navigator will provide navigation services that are culturally attuned and responsive to the people of the community being served delivered in a trauma-informed  and respectful manner. The HEART Family Navigator must be able to communicate effectively and utilize a strength-based approach and follow current best practices that are supportive to cultural, gender identity and sexual orientation needs of the family. Additionally, the Family Navigator shall be knowledgeable of and highly connected to the environments and systems through with the youth/parent/family must move in order to obtain services.

Requirements

RESPONSIBILITIES: 

  • Collaborates effectively with a multi-disciplinary team which includes, but not limited to: Latino Network, Clackamas County Juvenile Department, Clackamas County Schools and other stakeholders/partners.
  • Demonstrates leadership in program development and organization.
  • Assists youth and families to identify and utilize appropriate community resources, public services, and general support systems.
  • Facilitates and co-facilitates teaching circles, skill building groups
  • Streamlines tasks, appointments and paperwork.
  • Applies restorative justice training, practices and values to daily client interactions and recommendations.
  • Develops and maintains professional and trusting relationships with youth and families that encourage positive social and emotional development. 
  • Works collaboratively with youth and family to address their full range of needs by establishing short-term and long-term intervention goals for each youth using a strengths based, client centered, Youth Development approach, and utilizes a service plan that includes a list of needs of each client and plan for meeting each need. 
  • Makes referrals based on appropriate match between youth's needs and available resources. Track interventions and outcomes.
  • Advocates with referral agencies as needed and when appropriate to facilitate referral for identification of client needs. 
  • Exhibits an awareness of commonalities and differences among youth of diverse backgrounds and shows respect for those of different talents, abilities, sexual orientation and faith. Builds on diversity among and between individuals to strengthen the program community and the community at large by serving as a role model for the principles of inclusion and tolerance. 
  • Maintains punctual, regular and predictable attendance.
  • Works collaboratively in a team environment with a spirit of cooperation.
  • Other duties as assigned.

SKILLS, KNOWLEDGE, EDUCATION AND EXPERIENCE

  • High School diploma or higher and/or a minimum of two years equivalent experience in community outreach or social services.
  • Knowledge of emergency, social, education, domestic violence, child welfare, human services, and information/referral systems.
  • Excellent written and oral communication skills in Spanish and English (including public speaking).
  • Ability to employ critical thinking and confidence in decision-making and carrying out actions.
  • Proven ability to work under pressure, manage time and prioritize tasks to meet goals and deadlines effectively.
  • Demonstrated ability to work independently, and as a collaborative team member.
  • Commitment to personal and professional development.
  • Knowledgeable trauma informed practices with youth and families who are justice involved.

The employee is frequently required to stand; walk; sit and stoop, kneel, crouch, or crawl and spend the majority of time in an office environment. They may be required to perform minimal heavy lifting and walking during peak events and prolonged periods sitting at a desk and working on a computer. 


WORK ENVIRONMENT

While performing the duties of this job, the employee is frequently exposed to wet or humid conditions (non-weather); outdoor weather conditions; extreme cold (non-weather), and extreme heat (non-weather). The noise level in the work environment is usually moderate.
