POSITION TITLE: Family Navigator- School Based
REPORTS TO: Youth Empowerment & Violence Prevention Manager
CLASSIFICATION: Full Time, Non-Exempt, 1.0 FTE
LOCATION: Clackamas, OR
COMPENSATION: $23.07 Hourly

POSITION DESCRIPTION
RICH (Restoring, Individuals, Community, and Hope) is a partnership between Latino Network and the Clackamas County Juvenile Department. This program utilizes restorative justice values and principles to restore hope and resiliency in youth and families. The RICH SBIRT Family Navigator will play an integral role in navigating youth and their families to resources as informed by the School Based-Screening, Brief Intervention, and Referral to Treatment and Services (School-Based SBIRT) program. This is a partnership between Latino Network, Gladstone School District, Reclaiming Futures, and Clackamas County Juvenile Department.This program aims to increase wellness in the youth and families by responding to youth needs as reported by the Check Yourself Screening Tool. The Family Navigator will work alongside school staff, Clackamas County Juvenile Staff, Juvenile Intake and Assessment Center (JIAC) staff and Latino Network staff to provide family navigation services that are culturally attuned and responsive to the people of the community being served. Family Navigator must be able to communicate effectively and utilize a strength based approach. Additionally, the Family Navigator shall be knowledgeable of the environment and system through which the youth/parent/family must move in order to obtain services and that is highly connected.
RESPONSIBILITIES
  • Collaborate effectively with a multi-disciplinary team which includes, but not limited to: Latino Network, Gladstone School District, Clackamas County Juvenile Department, Reclaiming Futures, and other stakeholders.
  • Collaborate with stakeholders, school counselors, mental health professionals, and Latino Network staff to educate the community about the services provided by the SBIRT Family Navigator.
  • Utilize motivational interviewing skills, restorative justice practices and values when interacting with youth and families.
  • Attend all meetings related to RICH SBIRT program.
  • Implement the School Based-SBIRT model to fidelity.
  • Track interventions and outcomes.
  • Facilitate and co-facilitate skill building circles.
  • Conduct Initial Assessments to identify the youth's presenting issues and determine their strengths and needs with attention to immediate and mid-term service provision.
  • Develop and maintain professional and trusting relationships with youth and families that encourage positive social and emotional development.
  • Create and nurture Protective Factors (caring relationships, high expectations, and relevant and meaningful participation) in each youth's environment, including within the multi-agency team setting.
  • Work collaboratively with youth and family to address their full range of needs by establishing short-term and long-term intervention goals for each youth using strengths based, client centered, Youth Development approach, utilizing a service plan that includes a list of needs of each client and plan for meeting each need.
  • Make referrals based on appropriate match between youth's needs and available resources. Advocate with referral agencies as needed and when appropriate to facilitate referral for identification of client needs.
  • Exhibit an awareness of commonalities and differences among youth of diverse backgrounds and show respect for those of different talents, abilities, sexual orientation and faith. Build on diversity among and between individuals to strengthen the program community and the community at large by serving as a role model for the principles of inclusion and tolerance.
  • Maintains punctual, regular and predictable attendance.
  • Other duties as assigned.

Requirements
SKILLS, KNOWLEDGE, EDUCATION AND EXPERIENCE
  • High School diploma or higher and/or a minimum of two years equivalent experience in community outreach or social services.
  • Knowledge of emergency, social, education, domestic violence, child welfare, human services, and information/referral systems.
  • Excellent written and oral communication skills in Spanish and English (including public speaking).
  • Ability to employ critical thinking and confidence in decision-making and carrying out actions.
  • Proven ability to work under pressure, manage time and prioritize tasks to meet goals and deadlines effectively.
  • Demonstrated ability to work independently, and as a collaborative team member.
  • Commitment to personal and professional development.
  • Ability to regularly work in the community and in the office.
  • Evening and weekend availability may be required, and work hours are flexible to accommodate program implementation.
  • Reliable transportation required. Automobile, driver's license, and automobile liability insurance preferred; must be eligible to be an insured driver under Latino Network's Commercial Auto Liability (or become eligible within three months of date of hire).

May require heavy lifting and walking during events and outreach activities.
WORK ENVIRONMENT
While performing the duties of this job, the employee is frequently exposed to wet or humid conditions (non-weather); outdoor weather conditions; extreme cold (non-weather), and extreme heat (non-weather). The noise level in the work environment is usually moderate.
