Job Summary:
The Health Promotion Center (HPC) Nurse Practitioner is responsible for seeing 8-14 patients per day in a primary care clinic in City Heights with a primarily Spanish speaking patient population. Functions with minimal supervision to perform physical exams, new patient visits, and follow up visits that are age-appropriate. Responsible for taking thorough histories and completing appropriate physical assessments; ordering diagnostic tests and referrals as appropriate; and interpreting results and reports. In charge of completing preventative and disease related education with patients as well as prescribing appropriate therapies for identified health problems. Responsible for furnishing medications as appropriate for patient diagnoses and monitoring for appropriate therapeutic effects. Responsible for referring to higher levels of care and specialists as appropriate.
Salary Pay Range: $53.68 - 61.60 per hour

Job Description:
RESPONSIBILITIES
- Works in conjunction with other HPC NPs
- Works with HPC team to help plan flu vaccination events and community outreach events as needed
- Aids in precepting nursing (Undergraduate and Graduate level) students completing rotations at the HPC
- Aids in the supervision of graduate level NP students
Duties/Responsibilities:
- Complete patient visits in a timely manner
- Complete charting on all encounters within 1 business day
- Maintain appropriate documentation in medical records of health encounters and other follow-up communication.
- Work well within the Health Promotion Center team of providers and externs
- Maintain Standardized Practice Guidelines as specified by the Board of Registered Nursing (BRN) of California.
- Consult with HPC supervising physician and other HPC providers when health care practice issues arise or when treatment plans are not producing expected response.
- Assist patients with Project Access Applications and Management
QUALIFICATIONS
Required Skills/Abilities:
- Proficient or fluent in Spanish (able to conduct patient visits in Spanish)
- Proficient in Microsoft, Excel, Google Drive
- Able to learn new software and Practice Fusion charting system
- Excellent written and verbal communication skills
- Excellent skills in patient history taking, physical assessment, and management of a variety of chronic and acute health conditions.
Required Education, Experience, and Licensure:
- Masters or Doctoral prepared Nurse Practitioner
- NP CA license, NP CA Furnishing License, NPI
- National NP Certification (ANCC or AANP)
- CPR Certified (BLS required)

PHYSICAL REQUIREMENTS
- The work is primarily sedentary and may require sitting for extended periods of time. Use of computer equipment: computer keyboard, mouse, and monitor.
- Prolonged periods of standing during patient evaluations
- Ability to read documents, email and other correspondence, and reports on paper and computer monitor. Hearing and speaking to communicate effectively with others in person and by phone. Ability to stand, stoop, push, pull, and lift up to 20 lbs throughout the day.
