JOB SUMMARY: Under the direct supervision of the Program Manager, the Health Educator is responsible for outreach, recruitment, and providing prevention workshops, focusing on adult women identified as high-risk for STI/HCV/HIV, and substance misuse. The Health Educator will deliver evidence-based interventions such as Parenting Journey and WILLOW in a group setting. In addition, the Health Educator will collaborate with team members to support individual sessions and referral follow-ups for Navigation Services
DUTIES AND RESPONSIBILITIES
- Develop and implement health education programs for individuals and groups.
- Deliver the program’s high-impact evidence-based interventions such as Parenting Journey (36 clients -432 encounters)
- Run a WILLOW support group (14 clients - 70 encounters) and conduct all phases of the program curriculum as outlined.
- Collect and analyze data to assess the needs of the community.
- Conduct motivational interviews with patients to encourage healthy behaviors.
- Deliver presentations on health-related topics to the public.
- Attend outreach events, health fairs, tabling, etc., and recruit the population of focus for enrollment into the program.
- Create linkages and partnerships with other providers to recruit the target population.
- Attend weekly outreach and social media meetings. Outreach events may include some evening and weekend schedules.* (12 internal events and 9 external)*
- Creates content for Social Media posts and works with 12 posts per year (1 post per month reaching 5 people for each post)
- Other duties as assigned based on program needs.
KNOWLEDGE, SKILLS, AND ABILITIES:
- MINIMUM: Two (2) years in group facilitation, or providing education in a similar environments.
- Previous work experience with high-risk, at-risk, and HIV+ populations
- Ability to conduct HIV Testing and Hepatitis C, a plus
- One (1) year conducting case management with the target population.
- Exceptional written and verbal communication skills, including effective public speaking and interpersonal skills
- Ability to collaborate with internal and external stakeholders.
- Proficient in Microsoft Office (Word, Outlook, Excel, PowerPoint, etc.)
- Ability to withstand extended sitting, extended standing, stairs, and extended travel with or without reasonable accommodation.
- Must be willing to work weekends and long/flexible hours as required by the position

EDUCATION
- MINIMUM: High School Diploma/ GED with at least three (3) years of professional experience in the field or
- Bachelor’s Degree in health education or related field and one (1) year of relevant experience
WORK EXPERIENCES:
- MINIMUM: Two (2) years’ experience working with the population of focus
- Must have parental experience or status.
- Strong outreach Skills, including creating partnerships,
- Experience in group facilitation, comfortable with discussion/teaching classes.
- Familiar with using EHR systems, plus
- Effective communication, including writing, speaking, active listening, and presenting
- Up to 50% of traveling required.
LICENSURE: A NYS Driver’s License in good standing, preferred.
