Position Summary


Responsible for assessing the needs of assigned youth related to Life Skills, providing support, mentoring and materials that support their growth and goal attainment. The relationship with the youth will help the youth build a foundation for a successful transition to adult life outside of dependent care.

Principal Duties and Responsibilities

  • Maintain a caseload of youth in the Supervised Independent Living Program.
  • Meet with each youth according to contract standards; and in any case, at least twice a month; one of those visits occurring within the youth’s residence.
  • Initiate discharge planning for the youth from the outset and work towards successful aging out.
  • Perform assessments and assist youth with developing goals towards independence.
  • Provide mentoring, modeling and materials to assist with the learning and application of Life Skills for the youth.
  • Analyze client needs and develop appropriate interventions.
  • Perform tasks related to monitoring Life Skills.
  • Interact with collaborative service providers in order to meet the client’s needs.
  • Attend court hearings and service planning sessions and any other meetings deemed necessary for the case.
  • Maintain accurate, detailed and current case records on each client; preparing all required documents according to established deadlines, as required by the contracts and scope of services.
  • Assume emergency duty as assigned, providing immediate assistance, including on-site presence during a crisis; relaying relevant information to the supervisor and program director and completing relevant documentation.
  • Support the healthy transition to a successful, independent adulthood.
  • Attend all scheduled staff meetings, supervisory conferences and in-service training sessions.
  • Relay summary information on each client to the Program Supervisor at regularly scheduled supervisory meetings and informally as necessary.
  • Work cooperatively with supervisor, peers, and clients.
  • Provide excellent customer service to Delta’s internal and external customers.
  • Demonstrate ethical and professional conduct at all times.
  • Participate in Delta’s continuous quality improvement projects and ongoing performance measures
  • Perform such other duties that may be assigned for providing excellent customer service and for the most effective operation of the company.


Essential Functions


  • Ability to read, write and speak English; perform arithmetical calculations
  • Ability to use excellent oral and written communications skills
  • Ability to use telephone, voice mail, copier, facsimile machine, computer and calculator
  • Ability to prioritize responsibilities and delegate duties to others
  • Ability work effectively with people, facilitating their growth and development
  • Ability to effectively engage older youth, primarily ages 14-21
  • Ability to use acquired knowledge of the problem of individuals living in the community and of resources for successful integration
  • Ability to operate vehicles with a valid driver’s license in accordance with applicable state laws, insurance provider requirements and company policies and procedures

  • A BSW or a bachelor’s degree or above from an accredited college or university in a social service field.
  • Prior experience in child welfare a plus.
  • Current Pennsylvania child abuse, FBI and criminal clearances and child abuse clearance from any state lived in the past five years.
  • Valid driver’s license and properly registered and insured vehicle.
  • Excellent interpersonal and communication skills.
  • Computer literate
