Overview
Health Coach - Population Management - Full Time Days Mon-Fri 830a-430p
The Health Coach will assist our designated patient populations in finding new ways to inspire and improve health habits. The position will work closely with patients to promote wellness, and helping individuals realize their personal best. The Health Coach will be responsible for developing a Health Coaching relationship with participants and assist them through the process of actively working towards better health by providing support, encouragement, and education. The Health Coach will work in a team-based environment partnering with nurses and other clinical staff to provide support for clinical issues. The Health Coach is responsible for outreaching to eligible participants, which will be identified using risk stratification systems and analytics, and enrolling participant in Health Coaching program and providing necessary Health Coaching to reduce or eliminate high-risk behaviors. They will promote patient self-management and empower patients/families to achieve maximum levels of wellness and independence. They will quickly assess and respond to client needs and issues. Help client work towards forming solutions for presenting problem. They should foster trust with participants to facilitate behavior change and encourage adoption of habits that are conducive to a higher quality of life. Reviews and distributes health education materials to identified to participants. Through collaboration with participants, develops customized care plans for program participants including self-management goals and plans of action. Develops and implements plans to increase client motivation and engagement and self-efficacy in all aspects of care. Documents all care management activities appropriately, promoting accurate and timely outreach and coordination of care. Serves as a liaison to providers, patients and families for coordination of services. Completes all tracking and reporting requirements for outcomes and evaluation. Refers to appropriate Population Health Outpatient Case Manager RN for clinical issues. Meet with Population Health Management Leadership regularly for patient updates, identify issues and develop strategies for resolution. Participates with community-based initiatives and collaborates with other health care and/or community organizations to facilitate effective care coordination. Maintains appropriate professional standards and provides appropriate follow-up for participants. Identifies opportunities for continuous improvement, develops related plans of action and implements process and documentation of improvements. Participates in regular team meetings and promotes collaborative team work. Participates fully as a member of the Population Health Management Team in delivering our mission. Demonstrates commitment to providing outstanding customer service in a manner that is reflective of our mission, value and credo.
Responsibilities
Core Responsibilites and Essential Functions
  • Patient Education * Contacts and performs initial interviews with individua who are eligible for Health Coaching programs * Provides necessary coaching to reduce or eliminate behaviors that are considered high-risk. * Distributes health education materials to patients * Coordinates with patients and other health care providers to provide coordinated care across the continuum * Provides reminders to patients about needed for preventative care, assists in scheduling and obtaining results to update health maintenance * Provides follow up when patients have not kept important referrals with physicians and specialists and other ambulatory appointments
  • Knowledge, Skills and Abilities * Demonstrates innovation and acceptance of new ideas and concepts * Ability to resolve conflicts patients have and assist them to overcome their objections about their health and behaviors * Optimistic and can motivate patients to make vital changes in their lives * Exhibit effective communication skills for working with patients, their families, and other healthcare providers. * Maintain good working relationship within department, WellStar and other community providers * Ability to manage conflict, stress, and multiple situations.
  • Documentation * Ensures chart notes accurately and timely reflects work and adherence to Departmental Standards * Ensures all records are up-to-date and legible. * Completes all Electronic Health Record software screens * Accounts for and indicates all services delivered in the Electronic Health Record * Participates in data collection. * onsults appropriate experts and uses appropriate resources and evidence to address practice questions.

Qualifications
Required Minimum Education
  • Bachelor's Degree in Exercise Science, Nutrition, Health Promotion or similar field. Required

Required Minimum Experience
  • Minimum 1 year experience working in ambulatory care, home health, physician practice, other community setting, or hospital in patient support or advocacy. Required

Required Minimum Skills
  • Ability to analyze, plan and think critically and problem-solve effectively.
  • Ability to organize, prioritize and be flexible, and work with a variety of teams.
  • Self-motivated, and ability to adapt to changing patient or organizational priorities.
  • Strong attention to detail.
  • Demonstrate and maintain high level of accuracy
  • Proven customer relations skills with patients with diverse backgrounds.
  • Knowledge of Population Health and Case Management processes.
  • Excellent organizational and professional communication skills both verbal and written.
  • Computer literacy, data entry, retrieval and report generation
  • Proficiency with Microsoft office suite products.
  • Knowledge of HIPPA, and maintain confidential information

Required Minimum License(s) and Certification(s)
  • National Board Certified Health and Wellness coach OR
  • Certified Health and Well-being Coach from WellCoaches
