Horizon PACE is excited to announce an amazing opportunity to be part of Horizon PACE’s program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare team at Horizon PACE will approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone. Horizon has over 25 years’ experience providing care in Kentucky and is the largest provider of adult day Health Care services in the state. Horizon is opening 3 PACE locations in Kentucky, and we are excited to expand services to older Kentuckians. Under the supervision of the ADHC Director, the Health Care Associate is responsible for working with indirect supervision from the Clinical Supervisor. Responsible for providing and assisting participants of Horizon PACE with activities of daily living, restorative and supportive care. Assists with organization and implementation of activity program. Monitors health status and provides a safe environment for participants. Functions as a member of Interdisciplinary Team.

Position Responsibilities:

  • Assists participants and or supervises with activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care and toileting) as assigned or needed.
  • Creates and participates in group(s) and one-on-one activities in accordance with daily activity calendar and as requested by the Director of Activities.
  • Monitors and assists participants with meals at feeding table.
  • Assists, throughout the day, in maintaining clean and orderly day rooms, kitchen and personal care areas.
  • Accompanies enrollee to medical and other appointments.
  • Performs and or assists with laundry, including clothes for participants.
  • Assists the Drivers with the loading and unloading of participants from vehicles. Travels with Drivers to act as an escort for participants as needed.
  • Demonstrates dependability through consistent compliance with scheduled work hours.
  • Participates in the activities of the Interdisciplinary Team as needed and carries out the plan of care as directed.
  • Notes changes in participant’s physical or emotional health and communicates this information to the appropriate staff person.
  • Maintains participants’ medical record and fulfills Horizon PACE charting and reporting requirements.
  • Assists the Transportation Department with participants’ takes home supplies.
  • Participates in and support Quality Improvement Initiatives.
  • Follows all Horizon PACE program policies and procedures and applicable Occupational Safety and Health Administration (OSHA) safety guidelines.
  • Acts within the scope of his or her authority to practice.
  • Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.
  • Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal Precautions.
  • Maintains safe working environment. Follows Horizon PACE Safety policies and procedures.
  • Participates in continuing education classes and any required staff and training meetings.
  • Performs other duties as required or requested.

Knowledge & Abilities:

  • Supports the Mission, Values, and Vision of Horizon PACE and adheres to the Corporate Compliance Code of Conduct.
  • Is knowledgeable of participant rights and ensures an atmosphere which allows for the privacy, dignity and well-being of all participants in a safe, secure environment
  • Supports, cooperates with and implements specific procedures and programs
  • Supports and participates in common teamwork
  • Assist supervisor with various tasks to support department operations as directed; may also be assigned special projects
  • Perform tasks which are supportive in nature to essential functions of the job, but which may be altered or re-designed depending on individual circumstances
  • Assists participants and caregivers in filing grievances

Experience & Education:

  • Preferred to have high school diploma or equivalent
  • Preferred Certified Nursing Assistant (C.N.A).
  • A minimum of one year of experience in working with the frail or elderly population preferred.

Job Type: Full-time

Pay: $15.50 per hour
