We are excited to announce our new role as a Health & Wellness Director at our beautiful newly renovated community at Markham House. New management have taken over and we're looking to hire new employees we can bring into our family at Markham. The Health & Wellness director will be responsible for overseeing the care and well-being of our residents, ensuring they receive high-quality, personalized care in a supportive and nurturing environment. This position will be eligible for bonuses.

Key Responsibilities:
- Resident Care Coordination: Coordinate and manage the daily care needs of residents, including assistance with activities of daily living (ADLs), medication management, and healthcare appointments.
- Assessment and Care Planning: Conduct comprehensive assessments of residents' physical, emotional, and social needs to develop individualized care plans. Regularly review and update care plans as needed.
- Staff Supervision and Training: Provide guidance, support, and supervision to care staff, ensuring they adhere to established care standards and protocols. Conduct regular training sessions to enhance staff knowledge and skills.
- Family Communication: Foster open and effective communication with residents' families, keeping them informed about their loved one's care plans, progress, and any concerns.
- Quality Assurance: Monitor and maintain compliance with regulatory standards and best practices related to resident care. Implement quality improvement initiatives to enhance the overall quality of care provided.
- Management: Respond promptly and effectively to emergencies or crisis situations involving residents, ensuring their safety and well-being at all times.
- Collaboration: Work collaboratively with other members of the interdisciplinary team, including nurses, physicians, therapists, and social workers, to ensure holistic and coordinated care for residents.
- Documentation: Maintain accurate and up-to-date records of residents' care plans, assessments, and interventions in accordance with facility policies and regulatory requirements.
Qualifications:
- Previous experience in a similar role within a senior living facility or healthcare setting.
- Familiarity with relevant regulations and standards governing senior living facilities (e.g., state licensing requirements, HIPAA).
- Has experience with EHR & EMAR softwares, ex Eldermark.
- Compassion, empathy, and a genuine desire to enhance the lives of seniors.
Salary $70,000 - $75,000 + Bonuses
