BronxWorks is seeking one Harm Reduction Specialist Case Manager to join a new, diverse and enthusiastic team at Bronx Point, a new supportive housing program located in the Bronx, NY. Bronx Point provides services and permanent housing to 82 formerly homeless households, consisting of both individuals and families with children. Reporting to the Assistant Program Director, the Harm Reduction Specialist/Case Manager has a primary role in overseeing all Harm Reduction programming at Bronx Point. The ideal candidate is knowledgeable about Harm Reduction treatment and training, willing to work with individuals and groups, and collaborate with onsite staff and resources in the community.


Qualifications

  • Credentialed alcoholism and substance use counselor (CASAC) or CASAC-T or equivalent and as specified by NYCDOMH
  • Bachelor’s degree in related field, such as mental health counseling preferred and years or experience in substance use field preferred.
  • Proficiency in English required; proficiency in Spanish preferred
  • Proficiency in modern business communications, including personal computers, electronic email, voicemail, facsimile and coper equipment

Essential Duties and Responsibilities
  • Provided direct services to client, including intake, engagement, assessment, service plan development, referral, advocacy, counseling
  • Manage case load
  • Monitor and document client progress toward service plan goals
  • Document all interactions with, or on behalf of clients
  • Maintain familiarity with program resource’s available on and off-site
  • Provide group and individual counseling
  • Coordinate client services with other social services, mental health, medical and other providers
  • Complete program reports on a monthly basis, or more frequently as required
  • Report to, and meet with supervisor on a monthly basis
  • Conduct home visits and other field work, as required
  • Perform additional duties as assigned


Program Specific Responsibilities

  • Conduct intake-assessments and reassessments
  • Maintain a caseload of clients including clients with mental health issues
  • Prepare service plans and service plan updates
  • Offer individual AOD (Alcohol and Drug) counseling & groups
  • Manage syringe exchange services
  • Track opiate overdose training
  • Cultivate and collaborate with Harm Reduction service partners in the community
  • Offer biomedical counseling which assesses the need for, discusses and provides education regarding the use of Prep and PEP and HIV Prevention tools for sero-discordant partners
  • Engage in coordination with staff client’s primary care provider to ensure linkage to and retention in care, get most recent lab results for CD4 and viral load and update HIV medications list
  • Offer evidence-based intervention such as Seeking Safety and Healthy Conversation in groups and individual sessions.
  • Enter services provided into documentation system such as E-SHARE, Salesforce
  • Perform additional duties as assigned.
  • Conduct home and field visits as approved.
  • Accompany clients to medical appointments.
  • Train clients to dispense Narcan.
  • Follow up regularly with clients.


Physical Requirements

  • Ability to use a computer for prolonged periods
  • Ability to occasionally lift and/or move up to 10 pounds
  • Ability to stand, walk, or sit for long periods of time
  • Ability to climb five flights of stairs, if required to conduct home visits and/or fieldwork
  • Ability to bend and retrieve objects and/or documents
  • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation
  • Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work
  • TB Test required with the first 120 days of employment’ for Shelters and Homeless Outreach programs
