We are currently hiring a Harm Reduction Outreach Specialist, to work in our Care Coordination Services Agency. As the Harm Reduction Outreach Specialist, you would be responsible for:
- Conduct prevention related Harm Reduction interventions with Project Safe Point participants
- Conduct safer injection related Harm Reduction interventions with Project Safe Point participants
- Maintain inventories of Harm Reduction supplies other than syringes
- Engage in participant recruitment activities such as outreach to community organizations and street outreach including participation in the Neighborhood Support Coalition Outreach (NSC) efforts
- Make initial contact with eligible participants through social contact referral.
- Provide on-going Risk Reduction Education, Safety Planning, and Treatment/Medication Adherence Counseling
- Document updates to electronic records, shared documents, and other formats as required by protocols, policies, and/or procedures
- Conduct field and home visits as appropriate, and monitor participant health status, access to services and quality of received services
- Implement Harm Reduction strategies aimed at helping individuals minimize their immediate health and safety risks.
Harm Reduction Outreach Specialist Minimum Salary: $38,000.00
Harm Reduction Outreach Specialist Maximum Salary: $38,000.00
Harm Reduction Outreach Specialist Schedule: Monday to Friday, 8:30 am to 4:30 pm
Qualifications:
- 2 year/ Associate Degree in health, education or human services OR RN, LPN, CASAC AND one year of outreach, CM or case work with persons with: HIV/AIDS or other chronic conditions, a history of mental illness, homelessness, chemical dependence, and/or other populations of persons in need. OR
- 60 credit hours of college study from a regionally accredited college or university or one recognized by the NYSED as following acceptable educational practices AND two years of CM or case work with persons with HIV/AIDS, a history of mental illness, homelessness, chemical dependence, and/or other populations of persons in need.
- Education requirements may be waived by the agency if candidate demonstrates more significant experience (3 years) in case management, care coordination or case management along with significant continuing education/training hours during previous
