GPD Veteran Housing Case Manager

First Step House is hiring a full time GPD Veteran Housing Case Manager in Salt Lake City, Utah!

The Veterans Housing Case Manager must be a Veteran or have experience serving Veterans, have a valid driver’s license, provide a Department of Motor Vehicle Record and be insurable through First Step House Insurance. It is expected that the Veterans Case Manager treat all Veterans served through this program with dignity and respect. The Case Manager has experience serving Veterans and/or special populations with disabilities, including substance abuse and/or mental health conditions. The primary objective of this position is to provide time-limited case management services to improve the retention of housing by Veterans who were previously homeless and are transitioning to permanent housing.

• Pay is $44,553.60 per year

Duties to include but not limited to:

• Screening Veteran’s for program eligibility.
• Attend coordinated entry and triage meetings for program referrals.
• Provide outreach to referred Veterans to get them enrolled in the program.
• Utilize Critical Time Intervention (CTI) practices and tools in implementing case management.
• Weekly 1 on 1 sessions to meet with Veteran clients for case management and housing plan progress and compliance.
• Make home visits to monitor housing stability of the Veteran clients.
• Provide educational activities related to meal planning, tenant responsibilities, the use of public transportation, community resources, financial management, and development of natural support for Veterans.
• Provide appointment management – Case Manager ensures support at whatever level is necessary to make sure clients sustain housing and meet all scheduled appointments including medical, mental health, legal, employment, housing, vocational rehab, etc.
• Refer clients to Vocational Rehab for education or vocational purposes when appropriate. Coordinate with Veteran and voc-rehab counselor when necessary to ensure Veteran receives full benefits of program. Assist with registration, Pell Grant, student loans, etc. when appropriate.
• Assist clients with realistic financial support options such as employment, job training, job search, schooling, grants, and disability benefits.
• Assist clients with all aspects of transportation needs such as bus passes, or FSH transportation in order to meet all scheduled appointments and emergencies.
• Administer exit surveys to clients when they complete services.
• Comply with Utah HMIS requirements.
• Gather appropriate releases of information to ensure compliance with both the Federal Health Insurance Portability and Accountability Act (HIPAA) and the Code of Federal Regulations (CFR 42 and 45).
• Attend all relevant employee training classes and staff meetings.
• Other duties as assigned.

Education and Experience:

• Experience working with Veteran’s is required
• Case management experience is preferred
• A minimum of a Bachelor’s degree in social work or related field is preferred·
• Experience in the substance-abuse field is preferred
