First Step House is hiring a full time Case Manager - Housing Specialist in Salt Lake City, Utah!

TheHousing Specialistserves clients that are engaged with First Step House services to find and maintain safe, stable and affordable housing. The goals of this position are to 1) provide housing location services for clients that are homeless or at risk of homelessness 2) develop and assist in executing a housing action plan (HAP) to eliminate barriers and obtain housing 3) Utilize progressive engagement to tailor services to the needs of the household to assist them in maintaining housing stability.

Duties to include but not limited to:
- Screen clients for program eligibility and assist them to complete eligibility paperwork
- Assist clients with obtaining background, credit and eviction information to identify housing barriers
- Assist clients with obtaining required documentation for programs
- Assist clients that are homeless and at risk of homeless with finding and applying for appropriate housing options
- Enroll clients in eligible HMIS programs
- Coordinate with landlords to complete required documentation and inspections for program eligibility
- Assist clients with move in process
- Perform regular individual case management sessions, including in-home visits
- Complete SPDAT and VI-SPDAT assessments
- Attend community triage to coordinate housing opportunities in the community
- Collaborates with outside agencies to connect clients with employment services, education resources, healthcare programs, housing programs, and other relevant community resources
- Collaborates with counselors, case workers, and outside agencies to deliver housing location services
- Uses internal tracking system to ensure proper, accurate, and timely billing
- Uses both Utah Homeless Management Information System (UHMIS) and Utah Web Infrastructure for Treatment Services (UWITS) database to enter case-management notes, client information updates, billing information and other relevant client information
- Gathers appropriate releases of information to ensure compliance with both the Federal Health Insurance Portability and Accountability Act (HIPAA) and the Code of Federal Regulations (CFR 42 and 45)
- Other duties as assigned
Education and Experience:
- Housing experience is required
- Case management experience is preferred
- A minimum of a Bachelor's degree in social work or related field is preferred
- Experience in the substance-abuse and or mental health field is preferred
