Full-Time Rn Home Health Nurse (Medicare)

Medicare Home Health Agency seeking an RN with Medicare Home Health experience.

A Medicare Home Health Nurse provides specialized nursing care to Medicare recipients in their homes. This role involves assessing patients' health conditions, administering medications, monitoring vital signs, educating patients and their families on healthcare management, providing nursing treatments such as wound care, foly care, and collaborating with healthcare professionals to ensure comprehensive patient care. Nurses must ensure compliance with Medicare regulations and document care accurately for reimbursement. Essential skills include clinical proficiency, strong interpersonal communication, and the ability to adapt to diverse home environments.

Requirements:

· Current state license as a Registered Nurse

· Minimum 2 years nursing experience

· Minimum 1 year Medicare Home Health experience

· Kinnser EMR system experience preferred

Responsibilities:

• Perform home assessments and procedures utilizing your extensive nursing experience and critical judgment skills
• Complete initial and ongoing assessments nursing care of all in home clients.
• Participate in ongoing interdisciplinary assessment of patients.
• Participate in the development and evaluation of the plan of care in partnership with the patient, representatives (if any), caregiver(s) and the interdisciplinary care team.
• Provide services that are ordered by the physician as indicated in the plan of care.
• Counsel and educate the patient/client and family in meeting healthcare related needs.
• Complete timely and accurate clinical notes including addressing the patient’s/client’s progress.
• Effectively communicates with other members of the interdisciplinary healthcare team providing care to promote coordination of patient/client care and planning for discharge.
• Communicate with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of care.
• Supervise paraprofessionals and licensed professional team members.

Qualifications:

· Must have strong Medicare Home Health experience

· Demonstrated OASIS knowledge

· Excellent knowledge of nursing care methods and procedures

• Excellent knowledge of emergency care
• In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.) and willingness to follow them at all times
