The role of the Home HealthLicensed Practical Nurse (LPN) involves delivering high-quality patient care and providing technical skilled services in the comfort of the patient's home.
- Deliver services within the defined scope of practice as per state regulations governing nursing practice, following the care plan, and collaborating with the healthcare team.
- Document patient progress after each visit and submit reports to the agency on a daily basis.
- Regularly assess and communicate the patient's nursing requirements. Educate the patient's family on caring for the patient and maintaining a healthy environment.
- Observe, record, and report the patient's response to treatment and any changes in their condition to the supervising nurse or physician.
Requirements for this position include:
- Active LPN license in the state of practice
- Valid Driver's License, auto insurance, and access to a reliable vehicle or public transportation
- CPR Certification is mandatory
