Full Time Home Health Patient Scheduling Coordinator for our Albany, Oregon office!

Work Schedule:
Monday thru Friday between 8:00AM - 5:00PM
Overview:
The Scheduling Coordinator is responsible for the coordination of patient/client physician orders, pre- authorizations and patient/client visits of physician orders, pre-authorizations and patient/client visits. Accurate and timely communication of scheduling changes between office and staff. Responsible for matching patient needs with staff talents and knowledge.

- Facilitates communications and works effectively between staff, patient/families, vendors, providers
- Completes scheduled work flow by assigning appropriate team members to planned frequency of visits
- Assist agency personnel in the care coordination of patient/client services
- Serves as a liaison between the organization, referral services, patients, clients and personnel
- Assists in identifying opportunities for additional/improved services and methods
- Actively participates in IDT meetings, department meetings, in-service, QAPI activities, and other related activities as appropriate
- Consistently represents Signature Healthcare at Home with integrity and professionalism to all internal and external customers
- Adheres to HIPAA regulations per Company policy and Federal regulations
- Performs other duties and activities as delegated by the Administrator/Clinical Manager
- High School diploma or equivalent. College degree preferred
- Minimum two years’ experience in administrative position in healthcare setting
- Minimum one-year experience in scheduling experience in healthcare setting
- Knowledge of medical terminology
- Demonstrates good communications, flexibility and cooperation in performing job responsibilities
- Possess a valid driver’s license and insurance in accordance with state and/or organization requirements
- Basic computer skills and knowledge of office technology
