Forward Service Corporation (FSC), a passionate, not-for-profit firm that strives to provide exceptional customer service in employment and training related services to local, state and federally funded program participants. Our non-profit firm is seeking one full-time FSET Case Manager to help those we serve achieve their dreams. The home office is located in Fond du Lac, WI.
The FSET Case Manager provides case management services by assessing applicant’s needs, making referrals and empowering individuals to reach self-sufficiency. The FSET Case Manager develops and maintains employer relations, community awareness and partnerships.
PRIMARY DUTIES INCLUDE
- Provide individual and group services to aid individuals in achieving goals
- Provide on-going case management, development plan and coordination with other facilities
- Enter data accurately and timely into appropriate networks
- Counsel individuals with strength-based, motivational employment and training
- Refer individuals for support services and other community resources
EDUCATION/EXPERIENCE
Associates degree and two plus years work-related experience and/or equivalent combination or education and experience.
COMPENSATION & BENEFITS
- FSC offers a very competitive wage for this full-time, non-exempt position with a minimum starting rate of $23.31/hour with additional compensation based on direct experience beyond the minimum requirements.
- Example: Meet only minimum requirements start at $23.31/hour; with 3 additional years of direct experience start at $23.66/hour; 5 additional years of direct experience $24.38/hour, etc.
