At Silver Creek Family Medicine the Patient Care Coordinator is the first point of contact for our patients/families, the familiar voice on the phone and an appointment coordinator. The work environment is a combination of multi-line telephone use, insurance verification, receptionist and scheduling coordination. This position involves assisting with the check-in and checkout processes. Gathers and records required information about patients and routes it appropriately. Provides specific customer services which include, confirming health insurance coverage and individuals obligations, obtaining necessary authorizations for care, scheduling appointments, and assisting patients in an office or clinic setting. This position requires out of the box thinking, strong ability to multi-task and above all else, exceptional customer service skills.
Patient Care Coordinator Job Description:
- Be at your desk and ready to answer phone calls at 8am
- Look up patients that do not have real time eligibility for insurance verification.
- Look up patients who have OHP to ensure they have a PCP assigned.
- Sorting RX requests and putting them in the assigned folder.
- Pre-register all appointments for the day. Make sure to fill in all of the information that you can before the appointment to make a check in process
- Register each patient as they present for a visit and follow the scheduling guidelines as outlined by the clinic.
- Determine the financial status of each patient, and ensure their insurance information is checked for active coverage and the card is scanned into the chart.
- Ensure that each patient pays a co-pay at the time of visit if that is necessary.
- ·If the patient has no insurance, make sure the patient is able to pay for their visit or set up a payment plan.
- Manage US Pay Collection and automatic payments.
- Assist patients in accurately completing appropriate forms and document all information into the chart as required (DCL, Sports Physicals, DMV)
- Sends out no show letters
- Processes medical records requests
- Schedule patients appropriately
- Answer all incoming calls to the clinic. Appropriately document calls received by patients, by scheduling them for an appointment or sending a clinical message to the appropriate person so the patient can get a response.
- Accurately document new patient requests from patients to submit to providers for approval
- Greet and attend to patients in person and over the phone.
- Receive faxed documents pertaining to medical and specialist visits, and routes them to the appropriate person in the clinic.
- Use the recall lists for patient appointments related to Wellness Visits and Well Child Checks.
- Professionally assist providers, staff, visitors and patients.
- Encourages patients to sign up for MyHealth.
- Responsible for patient registration, password assistance and proxy access.
- Technical troubleshooting.
- Perform closing procedures
- Balance the journal for the day and give to the billing department.
- Pull and count cash.
- Clean up the lobby and front area by wiping down surfaces.
- Reception team is able to leave when the last patient has left the building
- Check in with the back office before leaving.
