Are you looking for a full-time position in where you can make a difference in the lives of individuals experiencing mental health disorders who are also involved in the criminal justice system? We are looking for a Case Manager to join our forensic mental health treatment team in Hamilton, Ohio.
Hourly Wage
$18 with a BA degree
$19.50 with a Master's Degree
The Forensic Case Manager works within the Butler Co jail as part of a forensic team assisting with discharge coordination
Case Managers serve as a community liaison aiding clients with accessing community resources. These resources include obtaining necessary identification, providing transportation to and from appointments, a liaison for court appointments, assisting with housing applications. This position works with individuals experiencing severe and persistent mental illness through our Transitional Living (TLC) agency.
This is a great starting point for a recent Social Work or Psychology graduate and students in a MSW or Master's in Mental Health Counseling program.
Qualifications:
- Bachelor’s degree in a social sciences field or related field preferred or 3 years of related social service experience
- Valid driver's license, car insurance, and insurable driving record required
- Communication, documentation, and organizational skills
- Customer service and/or social service experience a plus
Job Duties and Responsibilities:
- Works with individuals involved in the Butler County forensic system.
- Works with clients and multi-disciplinary team in an ethical, humane, culturally sensitive and respectful manner that executive that elicits collaboration and cooperation.
- Assist clients in obtaining services through community resources.
- Provides advocacy and assistance with accessing the health care system including help with scheduling medical and dental appointments.
- Collaborates and communicates effectively with other providers of service within and outside the agency regarding clinical care and individual client issues.
- Provides crisis intervention.
- Evaluates patient care plans with primary care team.
- Maintains appropriate documentation and written and/or verbal reports including our Electronic Health Record.
