Responsibilities:

• Conduct comprehensive assessments and evaluations during homecare visits to ensure high-quality care delivery.
• Manage admissions, discharges, recertifications, and reassessments for clients in adherence to established policies and procedures.
• Collaborate with interdisciplinary teams to develop personalized care plans based on clients' unique needs, goals, and preferences.
• Oversee and coordinate the delivery of skilled nursing services, therapies, and necessary interventions for excellent client outcomes.
• Provide education and support to clients and families, empowering them in disease management, medication administration, and self-care techniques.
• Act as a liaison between clients, families, healthcare providers, and community resources for seamless continuity of care.
• Train and supervise homecare staff, ensuring compliance with standards of care, policies, and regulations.

Qualifications:

• Valid Registered Nurse (RN) license in the state of Minnesota.
• 2 years of experience in homecare or community health nursing.
• Demonstrated leadership and management skills, with the ability to inspire and mentor staff effectively.
• Excellent clinical assessment and critical thinking abilities, coupled with a passion for providing exceptional patient care.
• Strong communication and interpersonal skills to establish rapport with clients, families, and healthcare professionals.
• Proficiency in conducting thorough homecare assessments and developing comprehensive care plans.
• Reliable transportation and a valid driver's license with a clean driving record.
