Flexible Housing Pool- Case Manager

SGA provides a wide range of social services in some of Chicago's most challenged and consistently underserved communities and creates long-term and community-wide change. 

MAJOR DUTIES AND RESPONSIBILITES 

The Flexible Housing Pool (FHP) is a program of Chicago Housing Authority and DFFS to house vulnerable populations. The program is administered by the Center for Housing and Health, a CBO that is part of AIDS Foundation of Chicago. This year the Flexible Housing Pool added slots to serve youth who participate in SCaN. Through this contract, SGA will provide intensive case management to SCaN participants who are referred to SGA by the FHP. 

• Case Manager who supports a caseload of 20 individual clients or 15 families.  
• Conducting intake assessments for assigned clients and reassessments at six-month intervals. 
• Developing service plans; Setting and monitoring goals which are tenant driven. 
• Conducting at least monthly home visits 
• Providing and managing referrals and resources to meet tenants' basic needs, as well as psychosocial needs. 
• Ensuring tenants are connected to their community and social support. 
• Providing education to build tenancy skills to maintain housing stability. 
• Ensuring tenants obtain and/or maintain a source of income through eligible benefits or employment. 
• Ensuring tenants have insurance and are connected to primary care and other health-related services, as needed. 
• Coordinating the purchase of furniture, basic home items, and hygiene items upon move-in 

Requirements

QUALIFICATIONS FOR THE JOB

Education:

• Bachelor's degree in Social Work, Psychology, Criminal Justice or related field required.  

Experience:

• Experience working with both mothers, fathers and families. 
• Experience working with at-risk youth; Restorative Justice, Diversion and Violence Reduction 
• Experience in case management. 

Other:

• Sensitivity to cultural diversity is required. Ability to communicate and cooperate with diverse families, various professionals and community groups.  
• Strong ability to network and develop professional partnerships. 
• Ability to work flexible hours, including occasional evenings & weekends. 
• Ability to write routine reports and correspondence. 
• Ability to speak effectively before groups of clients or employees or the organization. 
• Exercises discretion and independent judgment in executing tasks. 

KEY COMPETENCIES

• Competent in Microsoft Office tools and inputting services in an online database. 
• Bilingual/Bicultural in English and Spanish preferred but not required. 
• Valid Driver's License and Automobile Insurance. 

PHYSICAL REQUIREMENTS

• Must be able to transport oneself to and from home visits. 
• Occasionally ascends/descends stairs. 
• Constantly positions oneself to assist children during activities. 
• Frequently moves desks, chairs, tables, easels, and children's toys.  
• At times may be required to lift up to 25 lbs. for various classrooms and events needs.  
• Frequently moves in outdoor weather conditions.
