We are seeking a Family Practice Physician to become a part of our clinical team in the city of Moreno Valley, CA!
The Physician is responsible for providing preventative, curative, and rehabilitative medical care to patients and determine appropriate regimen of health care for each patient’s care. Maintaining supervision of case management of Community Health Systems, Inc. patient’s including but not limited to: referral to appropriate specialists, ancillary services, arranging home care and hospital care as, call backs to pharmacies and patients as needed.

KEY TASKS & RESPONSIBILITIES
- Plan and provide the care for and the management of stabilized chronic conditions, asymptomatic and well patients of all ages and socioeconomic cultures.
- Assist in developing and implementing procedures and protocols to facilitate the delivery of comprehensive health services.
QUALIFICATIONS
- Must be a graduate from an accredited school of medicine recognized by the California Board of Medical Examiners; M.D.
- Must be Board Certified or Board eligible.
- Must hold a current license to practice medicine in the State of California.
- Must also have a current DEA certificate.
- One year of experience in Internal Medicine.
- Five years’ of work experience as a Physician and two years in management required.
- CPR Certification required and ACLS Certified preferred
- Fluency in English required; Fluency in Spanish is an asset for this role and is highly desirable
