Position Summary: Provides Case Management Services to pregnant and parenting women, infants and children to affect the delivery of health and supportive services in an integrated manner.
Will follow role assignments that are in alignment with the Patient Centered Medical Home Standards, and participate in all Patient Centered Medical Home efforts.
Essential Duties and Responsibilities:
- Maintains an assigned caseload.
- Conducts monthly or as needed Home Visits to each participant.
- Completes Individual assessments to determine the level of the clients health & service needs.
- Service planning (i.e. development of care plans) and resource identification as applicable.
- Links client to needed services; monitors service delivery and clients compliance.
- Documents clients encounters and data (Includes electronic health record and cornerstone systems documentation).
- Participates in community workshops, conferences, seminars, school programs and health fairs.
- Participates in client staffings and case conferences, as appropriate.
- Participates in identified staff development activities.
- Submits written weekly and monthly reports.
Non-Essential Duties and Responsibilities:
- Client identification through assigned outreach activities.
- Advocates on clients behalf to facilitate access to services.
- Coordinates or facilitates small health education workshops.
- Performs other related duties as assigned.
Education: Bachelors Degree (BSN or other related field in a behavioral science, social service or health-related area).
Required Licensure and/or Certification:
Minimum Experience: 2 years experience in community health or maternal & child health OR 1 year experience in child, family or community services. Good organization and problem solving skills. Typing and/or basic data entry skills. Proficiency in basic counseling skills. Knowledge of community resources and area service providers. Able to read, write and speak English. Good oral and written communication skills required.

Other Requirements: Saturday hours, evening hours and/or may be required to work at a different site.
