About the Role:
- Deploy to members’ homes, or wherever they are in the community, in response to requests for medical, behavioral health, maternal health, and social needs.
- Provide high-quality, person-centered, integrated, physical, behavioral, maternal, and social care to high-risk individuals in their homes as part of an integrated primary care team.
- Assist, in collaboration with our virtual urgent care team of APP’s and physicians, members with urgent care needs wherever they are in the community. Provide therapeutic intervention under the direct guidance of a physician.
- Work with an interdisciplinary community-based team, in collaboration with experienced APP’s, and under the direction of physicians, to facilitate primary care in the home and respond to the acute and acute-on-chronic needs of Cityblock members.
- Engage with members recently discharged from inpatient admission or recently seen in an Emergency Department to prevent readmission, as well as to educate members on the services and care and services available as a Cityblock member.
- In some markets, care for pediatric patients in their homes to prevent the need for Emergency Department visits.
- In some markets, assist pregnant or recently postpartum members with maternal health needs and navigation, as well as act as a support through pregnancy and delivery.
- Utilize Cityblock’s custom-built care facilitation platform, which will enable you to collect data, organize information, track tasks, and communicate with staff, patients, and family
Requirements for the Role:
- Certified [EMT-Advanced OR EMT-Basic] [Medical Assistant], NREMT certified and/or with an active license to practice in the state in which you are seeking employment with Cityblock.
- Drivers license and good driving record, [along with reliable transportation].
- 2+ years of field clinical experience as an [NREMT or AEMT] [MA], or combination of experiences related to direct patient care of an equivalent timeframe.
- Strong critical thinking skills, excellent communication skills, and strong management of complex clinical issues, providing effective handoffs and ensuring loop-closure.
- Experience and credibility to identify holistic opportunities to deliver clinical and social impact for each member you interact with.
- Ability to adapt and operate in a fast-paced, evolving work environment.
- A reputation for delivering excellent clinical service with follow-through and diligence.
- Strong, proven analytical skills, and aptitude for learning new technologies.
- A deep commitment to transforming health and social care for vulnerable populations.
- COVID-19 Vaccination is a requirement for this position.
