POSITION SUMMARY:


Under the general supervision of the Family Support Services Coordinator the Home Visitor will provide primary care management for families enrolled in the home-based option. He/She will be responsible for providing in-home instruction as well as a group socialization experience to parents and children (Pregnant and infants) which will enhance the parents' ability to fill the role of primary nurturer of their child. Will also maintain on-going contact with families and work with all other components (health, nutrition, family services, mental health, special services, parent education and resource/referral) to integrate services into the family setting. Must meet requirements per CPS Prevention Initiative and/or federal Performance Standards and any local, state and or agency policies and procedures. Caseloads will vary from 12 to 24 per Home Visitor, based on the funding source.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:


1. Responsible for assisting with selecting, developing, and preparing appropriate
instructional material for the family.

2. Responsible for cooperating with other members of the staff in planning and use of
instructional materials and in the performance of their duties.

3. Responsible for providing his/her own professional growth through an ongoing
program of reading, workshops, seminars, conferences, and/or advanced course work at institutions of higher learning.

4. Responsible for assisting with planning weekly home visits, attending socialization
groups, coordination of periodic medical screenings and with center activities.

5. Responsible for conducting parent interviews and family needs assessments. c.
Conducting developmental screening and assessments.

6. Responsible for providing families with developmentally appropriate activities and
information designed to enhance the child's intellectual, social-emotional, motor, and
language abilities.

7. Responsible for assisting with recruiting and enrolling eligible families in the program.

8. Responsible for providing program parents with guidance on home safety, nutrition, effective discipline, constructive play activities, and other related topics.

9. Responsible for providing information on community and agency services and referring families to community resources to include medical and mental health providers and the interagency network for other services as needed.

10. Responsible for providing parents with information regarding childhood
immunizations.

11. Responsible for assisting with coordinating doctor, dental visit and other community needs identified by the families.

12. Responsible for providing designated staff with information regarding home visits,
screening and results, community services and referrals, resources and family needs.

13. Responsible for working with support services team members, staff, health, and/or social services in the performance of their duties.

14. Responsible for participating in the enrollment, recruitment, screenings and staffing of children.

15. Responsible for making provision for being available to families for education-related purposes outside the instructional day when required or requested.

16. Must attend and participate in monthly group socialization meetings with families and for establishing and maintaining open lines of communication with families concerning the intellectual, social, emotional, physiological and maturational growth of the child.

17. Responsible for establishing and maintaining cooperative relations with other
employees and service agencies.

18. Responsible for maintaining and promptly submitting accurate, complete, and correct records as required by federal/state guidelines, district policy, and administrative
regulations.

19. Responsible for assisting with maintaining an inventory of parent resource materials and coordinating family check-out procedures of resource materials.

20. Responsible for Completing clerical and accounting tasks necessary for the effective functioning of program activities.

21. Responsible maintaining individual records on children including screenings,
assessments, referrals, progress reports, documentation of home visitation activities, etc.

22. Other duties as assigned.

POSITION REQUIREMENTS:

  • Bachelors in ECE (Early Childhood Education) preferred or BA or BS degree in social work, human services, family studies or related sciences with course work that shows working with families and/ or social work or counseling in addition must have at least 15 college credits in (ECE) Early Childhood Education
  • Bilingual (Spanish) Required
  • Two (2) years related experience in outreach or casework
  • Must have valid Driver's License with proof of insurance
  • Knowledge of Illinois's Temporary Assistance for Needy Families (T ANF) regulations
  • Demonstrated ability to successfully engage and work with pregnant mothers, infants and their

families * Excellent verbal and written communication skills

  • Excellent computer skills
  • Demonstrates initiative and motivation
  • Preferred experience PAT model/curriculum
