Job Summary
Community Living Support Services -Assist with acquisition, retention, or improvement of skills related to home living in the community. Activities of daily living, personal grooming and health care, interpersonal relationships, mobility, home management, and use of leisure time.
· Social, leisure, and adaptive skill development that assists in continuing home living.
· Personal care, protective oversight and supervision.
· Training in activities of daily living, such as bathing, dressing, toileting, hygiene, light housework, laundry, meal preparation, and grocery shopping
· Medically related services, such as basic first aid, arranging, accompanying, providing individual transportation to medical appointments, and assistance with self-administration of medication.
· Implementation of behavioral support plan and to acquire new skills and behaviors.
· Transportation to grocery or personal shopping and other activities community activities supporting home living.
Community Access Individual Services -Assist Jacob with being involved in groups activities in the community that he enjoys and wants to be a part of.
Assist with learning and improving skills in personal care, being independent, socializing with others, adjusting to community life, using resources in the community, and becoming independent outside his home or family home. Assist to increase his skills in the community
· Increase contacts in community living.
· Improving daily living skills, leisure/recreation skills, communication skills, mobility, money management,
· Provide transportation to different places in the community
Responsibilities
- Know the ISP, HRST and Behavior Support Plan
- Meet with behavior specialist as scheduled.
- Track and record data related to behavior supports.
- Submit ABC data sheet for significant behaviors
- Follow daily schedule to the best of your ability.
- Utilize the visual schedule throughout the day.
- Assist with bathing and grooming: This includes shampooing hair, brushing teething, and Hibiclens shower or bath.
- Provide protective oversight. Client requires constant supervision.
- Food preparation: breakfast, snacks and lunch.
- Follow food plan
- Sit beside client while he eats to encourage good table manners and prevent choking. Provide prompts as needed for him to takes one bite at a time and chews thoroughly. The client tends to put too much in his mouth and does not chew his food well. He could easily choke.
- Assist with physical exercise which includes (but not limited to) swimming, walking, and playing at the park. The client needs to exercise regularly, weather permitting.
- Accompany client to medical and therapeutic appointments.
- Provide transportation to medical and therapeutic appointments.
- Take the client to the toilet every 60 - 90 minutes.
- Assist in collecting his medical supplies in the community.
- Assist with errands and shopping.
- Light housekeeping
- The client's ISP goals may involve the caregiver to assist in locations and activities that are not specifically listed in the job description.
- Provide opportunities for the client to use his communication device throughout the day to increase functional communication: greeting people, asking for what he wants or needs in an appropriate way, expressing emotions, and making choices.
- Medication Reminders
- Document anything unusual with client, such as crying, prolonged teeth grinding, loss of appetite, fever, cough, diarrhea, constipation, excessive pushiness, aggressive behavior, agitation or sleeping during the day in the daily notes.
- Report any bruises, scrapes, bumps, scratches under daily body audit in the Daily Progress Notes
- Report anything unusual such as crying, grinding his teeth excessively, loss of appetite, fever, cough, diarrhea, constipation, sickness in the Daily Progress Notes.
- Submit daily progress notes within 24 hours of providing services. This is required before I can approve your hours.
Job Type: Full-time
Pay: $20.00 - $22.00 per hour
