Job Description:
The Division VP of Case Management fosters effective communications amongst the division, hospitals, and all disciplines. The Manager champions HCA strategy and company/division/facility initiatives to ensure short and long-term objectives operationalized within the division. The Manager will identify opportunities for improvement and facilitate process standardization to promote improved outcomes, maximize resource utilization, and improve organizational efficiencies. Supports the HCA standards and objectives through ensuring effective care coordination, Case Management, and transitions of care processes at the hospitals in the division. Works directly with facility CM leadership and staff to provide support as needed.
Roles and Responsibilities:
• Providing support to facility directors and staff, where needed, for daily operations.
• Assesses Case Management performance in division hospitals and identify improvement opportunities.
• Disseminates information accurately and timely with multiple audiences, ability to present to senior team leaders at the hospital and division level, as requested with involvement in measurement outcomes.
• Stays knowledgeable regarding practice standards & CMS regulatory requirements.
• Supports the implementation of regulatory requirements, HCA Ethics and Compliance policies, and quality initiatives related to Case Management services.
Qualification/Experience:
• Bachelor's degree Nursing Required
• Master's degree in Nursing, Health Administration, or Business Administration Preferred
• 3+ years overall acute care HCM Required
• 3+ years in Case Management leadership Required
License/Certifications:
• Registered Nurse - Currently licensed as a registered professional nurse in the state in which he or she practices
• Certification in Case Management Preferred
