NaphCare is hiring an experienced Discharge Planner just like you to join our team at the Clackamas County Jail located in Clackamas, OR. Full Time Day Shifts are available!Pay rates range between $26.00 - $30.00 per hour based on Shift, FT, site needs, and experience.

NaphCare has a partnership with NetCE that provides CEU/CME for our staff. NetCE uses a rigorous peer review process to ensure that all activities and content are up-to-date. This service streamlines continuing education for all NaphCare Employees to meet state specific requirements for maintaining licensing.
Position Description:

• Conduct rounds with physicians to meet with patients to discuss placement orders
• Assist patients with a specific plan for medication management
• Include patient in discharge planning
• Assist patients with the identification of a support network, instructions for future care and follow-up
• Coordinate care with community agencies, shelters, employment agencies, and drug and alcohol counselors
• Bachelor's degree or a clinical license
