·Founded in 1952, Horizon House, Inc. in Philadelphia, PA is one of the largest providers of behavioral health, homeless and intellectual disability services in southeastern Pennsylvania and the State of Delaware. With our continued growth, the Horizon House Developmental Services (HHDS) Division has an exciting opportunity for a Director of Service Coordination. Working with the HHDS Chief Service Officer and Management Team, our candidate will oversee the growth and development of Service Coordination Services as the landscape of long-term residential services for Developmental Disabilities and Autism changes in Pennsylvania. The individual will:
- manage the clinical and Service Coordination teams and has overall operation and budgetary responsibility for the Service Coordination and case management services
- Represent Horizon House to other service providers and community groups to support the needs of residential services.
- Develops, revises and implements programs, policy and procedures in the Service Coordination
- Promotes professional growth/development and provides supervision/leadership of 5 Program Specialists and 1 Care Manager.
- Develops and provides Service Coordination training curricula aligned with best practice and reviews employee output to improve performance
- Interpret and understand data analytics that measure efficiency.
- Identifies process improvement opportunities and develops, implements and monitors action plans based upon identified individual and departmental metrics
- Be available as a resource to staff providing consultation as needed to immediately address and problem solve participant and service-related situations as they arise.
- Ensures compliance with state, federal and relevant regulatory bodies
Qualified candidates will have:
Minimum qualifications include: Master's level healthcare professional (e.g., LMSW, LMHC) with 3years experience supporting individuals with a diagnosis of intellectual or developmental disability and or Autism and 5 years of progressive management experience supervising clinicians and/or Case Managers who are providing direct services to individuals with developmental disabilities
OR,
Bachelor’s degree in Social Work, Health Care Administration, related field or equivalent experience with 5-7 years of social work, healthcare administration experience and 7 years of previous management experience including responsibilities for hiring, training, assigning work and managing performance of staff. Current, valid professional license is required
