Director of Home Care and Navigation - Full Time Position (Exempt)

The Director of Home Care and Navigation is responsible for the programming designed to meet the needs (including emotional, environmental, health services, intellectual, physical, social, spiritual and vocational dimensions of wellness) of Residential Living residents as well as directing and organizing the daily operations of the Home Care department which includes evaluating residents’ needs and assisting residents in navigating the available services that create the bridge between independent living and nursing care. The Director is responsible for ensuring that Independent Living residents are residing in a safe environment and helps coordinate a seamless transition between appropriate levels of care offered at the community through the direction of their team.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
  • Directs the navigation through levels of care for Residential Living residents and Home Care program’s operation by providing day-today oversight to ensure standards are being met for delivery of these services.
  • Completes the initial assessment of residents as they are getting ready to move into community and ensures that appropriate level of Care and services are in place to meet residents’ social, physical, and emotional needs.
  • Builds relationships, solves problems, and locates resources for Residential Living (RL) residents transitioning throughout the continuum of care.
  • Reviews all incident and security reports and coordinates appropriate follow up based on events that have occurred.
  • Evaluates residents’ needs and assists residents in navigating the available services that create the bridge between independent living and nursing care.
  • Ensures residents are in the appropriate levels of care (Residential, Assisted, Home Care, and Skilled Nursing) within the community and are receiving the supportive services needed to obtain optimal levels of health.
  • Develops and maintains listing of personal service providers (PSP’s) in the community and ensures tracking of required documentation.
  • Assists residents with understanding of their specific health plan benefits.
  • Conducts in-service training for team members on abuse prevention, PSP policies, levels of Care and other job-related topics as needed.
  • Participates in and prepares reports for the Quality Assurance and Performance Improvement (QAPI) meetings.
  • Assists in development and implementing written policies and procedures that reflect the objective and philosophy of the program.
  • Serves as chairperson for the campus HIPAA committee.
  • Interviews, selects, trains, coaches and evaluate Home Care team members.
  • Prepares and submits timely reports as required including but not limited to variance reports, aged reports, month-end reports, overtime reports, and government agency reports.
  • Monitors and ensures nursing assessments are being completed according to regulatory requirements.
  • Creates and initiates a business marketing plan to achieve 20% annual growth of Home Care services.
  • Develops and maintains listing of personal service providers (PSP’s) in the community and ensures tracking of required documentation.
  • Strives to achieve high levels of resident satisfaction and always looks for ways to improve resident’s experiences.
  • Develops, prepares, implements, and monitors the annual Home Care budget.
  • Review’s Home Care client financial records for consistency and accuracy in billing.
  • Oversees and manages the processing of long-term care or supplemental insurances for resident clients.
  • Manages Home Care aged accounts and contacts residents/family members as needed for payments
  • Supervises the Independent Living Social Workers/Navigators as well as the community’s Home Care team to coordinate the following services for the Residential Living residents:
  • Helps resident and family to understand, accept, and follow medical recommendations, the necessity of additional services, or a physical move within the continuum of care.
  • Coordinates communication with physicians, families, and appropriate team members regarding residents’ status.
  • Maintains awareness and promotes all internal services (CV Home Care program, MD’s/NP’s in Clinic, Rehab program, etc.) to promote resident retention.
  • Works in close collaboration with physicians and other health care personnel in resident evaluation and treatment to further their understanding of significant social and emotional factors underlying resident's health problem. Facilitates communication with all key resources and stakeholders.
  • Assists in resident orientation to the community when needed on job related topics.
  • Continuous evaluation of the physical, emotional and/or social needs of residents within the community.
  • Coordinates and attends meetings related to resident transitions/status updates, including but not limited to:
  • Evaluation & Review Team (ERT) Committee (Chairperson of committee)
  • Care Coordination Meetings
  • Discharge Planning Meetings
  • Support Groups (plans and coordinates for residents, family members, and team)
  • Resident Health Service Committee Meetings (only when invited by committee)
  • Provides service planned to restore resident to optimum social and health adjustment within resident's capacity.
  • Utilizes community resources to assist resident to resume life in community or to learn to live within limits of disabilities.
  • Maintains resident records and status appropriate to navigate and communicate resident’s condition and/or health services.
  • Follows human resource policy and procedures, including but not limited to salary administration, performance management, employee relations, progressive disciplines, and compliance to government regulations.
  • Must interact courteously, tactfully and politely with managers, co-workers, residents, vendors or associates to preserve productivity and moral.

SUPERVISORY RESPONSIBILITIES Manages 2-3 subordinate supervisors who supervise a total of 100+ employees in the Home Care Department. Is responsible for the overall direction, coordination, and evaluation of this department. Carries out responsibilities in accordance with the organization's policies and applicable laws. Responsibilities for personnel actions include interviewing, hiring, performance management, and termination; planning, assigning, and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems. Must be able to evaluate and respond to the needs of staff, clients, and others.
QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Must be a self-starter, highly self-motivated, self-disciplined and able to work independently with a minimum amount of supervision dealing effectively with multiple priorities in a fast paced environment. Ability to prioritize and utilize good time management skills.
EDUCATION and/or EXPERIENCE Bachelor’s Degree (B.A./B. S.) in health, business, or public administration science and has at least one year of supervisory or management experience in Home Care or other licensed health care program. Preferred social work/Home Care experience in CCRC environment having a clear understanding and ability to assess needs of seniors aging in place. Must be CPR certified or ability to obtain once hired.
WORK SCHEDULE
Exempt position. Full Time Daytime hours, working at least 40 hours per week. Some additional hours will be required to meet the business demands of the position.
