Direct Support Worker (DSW) 

Summary

Work for persons with disabilities who are self-directing their care, assisting with activities and tasks according to their Plan of Care (may involve: activities of daily living, personal care, meal preparation/housekeeping in individual’s home). And may involve assisting person with grocery shopping, going to doctor appointments, and other errands as needed.

Essential Job Functions

1. Provide assistance with activities and tasks that are unique to a person’s disability needs in accordance to a Plan of Care
2. Promote activities and work on goals that will help a person remain independent and  in their home
3. Provide transportation as needed
4. Assist individual with appointments and activities in the community
5. Maintain a safe and clean environment in the person’s home
6. Stay in contact with SKIL Fiscal Agent (providing payroll services) regarding required documentation, such as hire packets, activity logs, time worked, etc.

Job Requirements

1. At least 18 years of age
2. Have a valid Driver's License
3. Have previous experience working in direct care
4. Clear background checks
