Current vacancy is with the Health & Human Services Agency located in Visalia. Provide direct field-based, telehealth (telephone or video), and office-based crisis evaluation, support, de-escalation, and safety-planning to those experiencing, or at risk of experiencing, a mental health or substance-use related crisis; provide comprehensive rehabilitative skill-building services to high-risk clients with a focus on crisis prevention, de-escalation, and safety-planning for individuals, and as needed as clinically appropriate, in a group setting and/or involving support persons; manage a caseload as a secondary coordinator for those at high risk; meet with clients primarily at the location where they are experiencing a crisis or need support to prevent a mental health or substance use crisis, which may include but is not limited to a client's home, school, or workplace, in the community, at clinics, hospitals, other treatment facilities, and unhoused habitations; assist with use of screening and assessment tools at the paraprofessional level to identify biopsychosocial determinants of health impacting the client's health and general mental well-being; identify and provide support for risk or signs of drug-related overdose to include, but not limited to, psychoeducation, administration of Naloxone, and/or coordination with emergency medical services; assist clients in accessing health, mental, social-related services, and community resources, such as referrals, accompaniment to specialist appointments and assistance with enrollment forms as needed and transporting clients when safe and appropriate; short-term follow-up check-ins following a crisis assessment and response; ensure timely client contact and timely documentation submission, maintaining a variety of progress notes, service records, and other required documentation in standard form and language; provide documentation of consumer services and encounters following all federal, state, and local guidelines and policies and procedures; make reports concerning family situations involving suspected neglect or child abuse, or abuse of an elder or dependent adult in accordance with mandated reporting requirements; work closely with law enforcement and may be assigned to field-based collaborative and/or ride-along work with contracted partner agencies, mobile units, or law enforcement; supports, as needed, as a consumer liaison to work alongside law enforcement to safely intervene and effectively divert individuals in crisis during a police encounter; contribute to multi-disciplinary treatment team discussions; maintain confidentiality of all information and materials handled; gather, disseminate, and coordinate relevant information, strengths and needs, necessary to connect individual to necessary services and resources; work collaboratively with all Agency behavioral health teams, community physicians and health providers, social workers, probation officers, Tribal partners, Regional Centers, Adult Protective Services (APS), Public Guardian, and community-based organizations; participate in risk reviews of specific cases for evaluation, discussion of specific intervention techniques and overall resolution effectiveness while reviewing alternate courses of action that may have been employed; participate in consultation, education, and information in preventing, deescalating and resolving crisis and in response to requests from community agencies, individuals, and professionals for information and education about behavioral health; perform various keyboarding and specialized general clerical duties to complete assigned work, prepare and maintain case records, case narratives and court reports; assist in outreach and developing new community resources; transport clients to and from mental health clinics and community-based resources, including emergency shelters while abiding by safety practices; provide shadowing support and mentoring to interns, volunteers, and new employees; assist individuals in linking with organizations and resources to address underlying conditions contributing to a crisis such as basic needs (food, shelter, clothing) and activities of daily living (personal hygiene, medical care, finances).

Education: Bachelor's degree in social work or closely related field.
Education Equivalency: Caseload management experience equivalent to the level of a Social Services Worker III may be substituted for the educational requirement on a year-for-year basis.
Knowledge of: Social aspects and characteristics of mental and emotional disturbances and substance abuse; dynamics of child abuse and neglect; interviewing and counseling methods and techniques; community resources available to assist clients; components of safety plans and their development and implementation; techniques for assessing family situations and problems sufficient to develop, implement, and monitor safety plans; mental health programs and community resources; principles and techniques of de-escalation and stabilization.; proper safety procedures and health practices; psychoactive drugs, contra-indications, signs of intoxication or withdrawal, and side effects.
Skill/Ability to: Work and communicate effectively with people of various education and socioeconomic backgrounds by respecting beliefs, interpersonal styles and behaviors of both clients and co-workers; operate contemporary office equipment inclusive of computer, keyboard, and all applicable electronic equipment; complete multiple priority projects with strict deadlines: assess and make referrals concerning family situations involving suspected neglect or child abuse; read, interpret, and explain rules, regulations, policies, and procedures about social service programs; adjust to changes in workload and coordinate work with others; use and develop community resources to best serve client's needs; act effectively with patience, tact, and courtesy in emotional and stressful situations to respond to emergencies and adopt an appropriate course of action; utilize sound judgment and take effective action without immediate supervision in consultation with other team members and with clinical direction from licensed staff; effectively engage with clients, develop, and maintain the confidence and cooperation of individuals experiencing mental health crises and/or under the influence or alcohol or other drugs; engage and establish rapport with clients using a variety of modalities; provide comprehensive rehabilitative skill building to effectively prevent and intervene with mental health and substance use-related crises; provide appropriate referrals and linkage of clients to community resources based on biopsychosocial needs; write concise case reports, case contact notes, and evaluations; maintain record-keeping and tracking systems of assigned clients; communicate effectively with community partners, including community-based organizations, law enforcement agencies, schools, and board and care operators; maintain confidentiality of client information.
License or Certificate:
Possession of, or ability to obtain, an appropriate, valid California driver's license
