What you’ll do:

The Crisis Navigation Facilitator position provides follow up services for adults and families in the community who are needing additional resources and support. They ensure that each client is connected to ongoing care with referrals to ongoing behavioral health services and community resources as needed. Hybrid position with a minimum of 3 full days in-office. Shift is Monday - Friday 8:00 am - 5:00 pm. *AZ residents only.

Essential Responsibilities:

• Act as the first point of contact for each CTN or other peer program referrals; each client should be outreached and provided resources and support.
• Schedule initial meetings for the CTN team members. Facilitator will also ensure services have begun and will provide additional support and resources if further needs arise.
• Responsible for clearly communicating information to CTN team member and Clients. Will complete all necessary discharge/closure paperwork in electronic record system within appropriate timeframes.
• Maintain follow-up database to ensure connection of care for all CTN programs and clients. All documentation into electronic record system must be entered within agency standards.
• Will complete monthly and weekly reports for all CTN programs.
• Act as a contact point for staff for further information needs.

What you’ll provide:

• High School Diploma/GED required.
• BHT experience preferred in the Behavioral Health field.
• Minimum of 4 years of experience working in the Behavioral Health Field or a Bachelor’s in a Behavioral Health related degree preferred.
• Experience in administrative or office based roles required.
• Experience collaborating with community resources.
• Experience collaborating with internal and external customers.
• Bilingual Spanish language ability preferred.

• Excellent oral and written communication skills.
• Detail oriented and strong organization skills.
• Able to be flexible with workload and availability, excellent time management skills.
• Strong internal and external customer service skills.
• Strong computer skills including experience with Microsoft Office.
• Ability to generate reports and analyze data.
• Demonstrated ability to remain calm in crisis situations, work independently and cooperatively, recognize personal limitations, and relate positively and with empathy to all persons regardless of age, race, creed, gender, or sexual orientation.
• Maintain a professional demeanor and positive working relationships with internal and external team members, stakeholders, and community and business partners.

What you'll experience:

• The typical work environment will be working indoors or community-based work, working out in the community.
• Employees must be able to lift up to 15 pounds at times.
• Occasional travel to training and out-of-state locations may occur as-needed.
