Responsible for providing perinatal assessments, educational services, and billing under CPSP guidelines and perinatal protocols, including the following:
1. Interview patients regarding various topics of sensitive nature in order to obtain relevant information for individual case management.
2. Provide support services to the “Growing Expectations Program” in all aspects of the program and any other programs that may fall under the department.
3. Assume duties of Health Educator as a substitute in their absence, not limited to teaching and/or facilitating classes for the CPSP, Growing Expectations Program, and Maternity tours.
4. Comply with CPSP policies set by Los Angeles County and the protocols set by the department and resources indicated within.
5. Attend all scheduled trainings relevant to the position including ongoing in-service training.
6. Promote to the patient the maximum program benefit and maintain a constant relationship with each patient throughout the course of the program components.
7. Create a positive and professional patient experience for each patient, creating an atmosphere of caring and support among staff and throughout program components.
8. Ensure that patient’s caseload charts are current and appropriately maintained.
· Appropriately document follow-up and physician instructions.
· Document information related to education and/or referrals given to patient on care plan.
· Communicate education and/or referral information with the attending Physician.
9. Responsible for submitting patient sensitive information before patients are admitted for delivery to ensure hospital staff is aware of relevant issues.
10. Provide Presumptive Eligibility (PE), maintain PE logs, and refer to Medi-Cal worker for scheduling appointments when appropriate.
11. Perform CPSP, Medi-Cal, WIC, and hospital tours.
12. Responsible for all contacts and referrals for patients.
13. Responsible for distribution and ordering of educational materials and magazines for patients.
14. Schedule appointments and/or follow-up with patients regarding new or missed appointments.
15. Ensure that all patients receive component of pre-admit, Medi-Cal, and Growing Expectations participation.
16. Ensure that all records related to coding of billing are current and accurate.
· Ensure that all billing and pre-admission documentation is submitted to CPSP admitting/billing coordinator before the beginning of the following workday.
· Acquire all insurance verification before submitting billing on a daily basis.
17. Correctly process changes in the daily schedule that affect patient appointments.
· Patient must be contacted and re-scheduled
· Physician must be notified
· Transportation arrangements, if applicable, must be rescheduled for the patient.
18. Work any/all special events and functions when requested which may include weekends, holidays, and time outside normal work hours.
19. Consistently demonstrate and uphold CCCHC’s principle of providing quality health and human services to the medically underserved and low-income populations in a culturally sensitive manner.
20. All other duties as assigned.
Special Knowledge, Skills, Abilities and Attributes:
· Reliable transportation, automobile insurance, and current CA Driver License.

· Required to know, follow and enforce safe work practices, and be aware of company policies and procedures related to job safety, including safety rules and regulations.
Education and Experience Requirements
- Bilingual (Spanish/English) required.
- High school diploma or GED required.
- Minimum two (2) years of experience in family planning preferred.
- Medical Assistant Certificate/diploma (Preferred)
