Community Outreach Services


Hiring for the following teams:
 

ICM
The Intensive Case Management (ICM) team is a mobile, community-based team who serves individuals 18 years on up who have been diagnosed with a serious mental illness. The ICM team has a smaller staff to client ratio given the client’s level of care needs. Cascadia’s ICM works both in Multnomah and Clackamas Counties however the ICM office is based in SE Portland.  The ICM team consists of the program manager, therapists, skills trainers, and peer wellness specialists. The team meets every morning for daily check-ins; ICM provides weekly group and individual supervisions. Cascadia ICM follows Strength Based fidelity practices; all staff are trained in Strength Based Management.  
 

ACT/FACT

Assertive Community Treatment (ACT) and Forensic Assertive Community Treatment (FACT) teams are an Evidence-Based Practice (EBP) designed to provide comprehensive treatment and support services to individuals who are diagnosed with serious mental illness.

Team members do not carry individual caseloads, rather the entire team is responsible for the entire caseload.  Team members meet daily to coordinate services for ACT participants. ACT teams are typically staffed at a ratio of 1:10 (meaning 1 full time team member for every 10 individuals served by the team). 

The ACT team includes: a team leader, a licensed medical practitioner (psychiatrist or PMHNP), nurses, therapists, substance abuse specialists, supported employment specialists, and peer support specialists. 

ACT meets clients where they’re at: Most services (80%+) are provided in the community where people live and work.  While ACT services are individualized to meet each person’s needs, because of the high acuity of ACT participants, they are typically seen on average of 4 times a week. 

 

Housing Outreach Team/ Palm2 PSH/ Street Outreach
Cascadia provides permanent supportive housing to approximately 240 households throughout Multnomah County through our Housing Supports and Services . These programs provide home and community-based services, including housing search and placement services, housing retention services, mental health services — including case management, skills training, counseling, medication services, and crisis intervention services. The Street Outreach Team actively works with unhoused individuals, providing resources, supplies as well as mental health services.

Duties:

  • Provide a wide range of therapeutic supports
  • Establish and assess level of care: complete behavioral health assessment, identify provisional diagnosis using current DSM, and develop services plans
  • Identify and assess for risk factors for harm to self and/or others in the context of the client's current psycho-social context
  • Provide continuity of care with other members of the treatment team and providers
  • Document and maintain clinical records and clinical progress
  • Advocate and refer to community and internal resources as necessary


Qualifications
