CONTRACTUAL PRP SPECIALIST / CASE MANAGER

PRP specialists have the opportunity to work at an Outpatient Mental Health Clinic with a Phenomenal and supportive team of professionals!

Seeking Contractual Team Members for our AA County location (Odenton, Glen Burnie, Severn and Forte Meade) but not limited to.

Candidates MUST meet the following requirements:

• Minimum of an associate degree in a public and/or mental health-related field or 30 hours or their equivalent of college credit in a public and/or mental health-related field.
• Additionally, prefers at least 2 years of work experience in a supervised public and/or mental health setting or its equivalent.
• Must have a valid driver’s license and be fully insured.
• Computer and adequate internet access.

Job Duties

• The PRP Specialist will be responsible for managing a caseload of minor and/or adult clients (minimum of 7 clients).
• The PRP Specialist will conduct a minimum of 3 visits per month for minor clients and 6 visits per month for adult clients.
• The PRP Specialist provides community case management services..
• The PRP Specialist will demonstrate expertise in implementing intervention services in support of the client’s Individual Rehabilitation Plan (IRP). This may require completion of and navigation of the social services process, housing resources and application process, and completion of mobility application assistance (this is not an all-inclusive list of duties).
• The PRP Specialist must complete client progress notes within a 24-to-48-hour time frame after each client encounter in the EHR system ICANotes.
• The PRP Specialist must complete a monthly collaboration with the client-therapist and other collaborative organizations.
• The PRP Specialist must complete a monthly summary for all clients within the required time frame.
• The PRP Specialist must attend the mandatory monthly “All Staff Meeting” and trainings as scheduled.
