Job Duties

The Department of Behavioral Health and Developmental Services (DBHDS) is seeking a qualified professional to work within the Office of Patient Continuum Services team to augment, implement and support the mission and vision of DBHDS. This position will work closely with the Commonwealth Center for Children and Adolescents (CCCA), Community Services Boards (CSBs), and the Department of Social Services (DSS), among other agencies, to provide assistance with census management, navigate access to the vast array of services and benefits across the spectrum of agencies, and support the discharge of individuals to appropriate community services, which allow the individual to live in the least restrictive and most integrated setting possible. The emphasis is to provide a strong regional liaison between DBHDS, CSBs, Regions and social service agencies; deliver ongoing support and technical assistance to CCCA, CSBs, and regions to ensure proactive discharge planning, operational compliance with the Discharge Protocol; and to support the effective utilization of discharge and diversion funding and other COV resources. This position assists CCCA, CSBs, and local DSS agencies (as applicable) to identify potential resources that are available for individuals with complex comorbid behavioral health and co-occurring disorders and serve as a liaison to private providers and organizations working with the child and adolescent population.
Additional responsibilities include:
Coordinating and collaborating with CSBs, social service agencies, nonprofit organizations, community partners and colleagues on the transition of individuals from CCCA to the community.
Provisioning of discharge planning with children and adolescents.
Nurturing strong relationships with private providers, professional associations, and CSBs.
Monitoring the ready for discharge and extraordinary barriers lists and responding to inquiries.
Maximizing effectiveness of discharge and diversion funding resources and providing spending oversight, requests for information, and assistance from internal and external constituents.
Preparation and analysis of reports, review of data to support ongoing quality improvement, and performing day-to-day administrative tasks that support Community Transition.

Minimum Qualifications

Experience with psychiatric hospital discharge planning for children and adolescents. Knowledge and expertise in contemporary best practices in behavioral health services and supports for children and adolescents with complex behavioral health needs and co-occurring disorders consistent with integration within the least restrictive and most integrated community setting. Specific knowledge and expertise in coordinating the optimum utilization of resources available through social service providers. Thorough understanding of the nature and process of recovery for individuals with mental illness, co-occurring disorders, and related conditions. Ability to apply recovery concepts and other constructs to service development, service delivery, and quality improvement approaches for children and adolescents with complicated behavioral health, co-occurring, and comorbid conditions. Ability to apply knowledge and expertise to support system development and transformation, monitor and support services and initiatives, manage projects to build and sustain constructive and collaborative relationships with private, nonprofit, public partners, and stakeholders. Ability to provide consultative technical assistance to partner entities. Knowledge of data-based performance and outcome measurements, quality improvement approaches and applications. Excellent oral and written communication skills.

Additional Considerations

Experience related to children and adolescents with behavioral health and/or emotional disorders, intellectual and developmental disabilities, along with complicated behavioral health, co-occurring and comorbid conditions, the challenges associated with these conditions, and best practices for maintaining quality of life in the most integrated settings required. Experience with Children's Services Act (CSA) and Family Assessment and Planning Teams (FAPT). Previous work with psychiatric hospitals, CSBs, and DSS agencies.
