Program Manager of inland Empire

In Lieu of Services- Community Supports Program

REPORTS TO: Director of IE ECM & ILOS-CS Programs

HOURS: Full-Time / 8:00am-4:30pm

CLASSIFICATION: Exempt

SUMMARY:

The Program Manager of Healthcare Programs will manage and assign caseloads and provide day-to-day program coordination for the ILOS-CS (In Lieu of Services-Community Supports) and ECM (Enhanced Care Management) Programs with the support and full knowledge of the Director of IE ECM and ILOS-CS Programs. The Program Manager of Inland Empire ECM and ILOS-CS Programs position will take the management role assigning and tracking program referrals to team members and provide status updates re: outreach and engagement, housing application assistance, intensive case management, linkage to IEHP, Molina, and Health Net care providers, and housing supportive services for members enrolled in the ILOS-CS and ECM Programs. The Program Manager of Inland Empire ECM and ILOS-CS Programs will also manage the Molina, IEHP, and Health Net TEL/referral lists, provide support to ensure the ILOS-CS and ECM Teams are meeting set enrollment goals, and ensure contractual and financial measures are met.

DUTIES: The following reflects essential functions for this job but does not restrict other tasks, which may be assigned:

Leadership, Administration, Oversight

· Coordinate: Team meetings, schedules for all members and ECM and ILOS-CS activities such as home visits, move-ins, pending intakes, documentation, and joint retention visits.

  • Manage: Once referred members are assigned, manage the outreach, engagement, enrollment, intake, assessments, and tracking of new enrollments for both ILOS-CS and ECM Programs.
  • File Compliance: Assist with file compliance and audit preparation in coordination with Director. Ensure as a Team responsibility all charts are in compliance at all times and audit ready in both paper and electronic versions. Report all non-compliance to Director for corrective actions.
  • Data Collection and Reporting: Ensure accuracy of all data, provide reports to Director as requested. Alert to inaccuracies and request assistance for corrections. Work with Program Assistant to develop monthly invoicing to ensure accurate invoices are submitted in a timely manner.
  • Track and provide reports as requested on housing placements for all housed individuals, as well as track enrollments for ECM and ILOS for IEHP, Molina, and Health Net, as well as ensure contractual and financial measures are met.
  • Assist with audits as needed.
  • Assist with all staff onboarding and training.

Direct Service and Support

· For member cases: Manage all field-based healthcare based and Community Support services that promote wellness and recovery for members that are transitioning from homelessness to permanent supportive housing. Facilitate and link members to appropriate IEHP, Molina, and Health Net resources.

  • Manage and conduct meetings with staff during weekly team meetings and individual supervisions with staff to review staff progress towards goals and reassess and revise service plans using recovery-oriented service models.
  • Manage staff and ensure educational and employment goals are being met during scheduled meetings.
  • Manage staff to ensure eviction prevention, initial and follow-up doctor appointments, and member goals are being addressed and accomplished.
  • Manage staff to ensure all outcomes are being met per ECM and ILOS contract requirements.

Documentation and Data Collection

· Work collaboratively with members of the ILOS-CS and ECM teams as needed to provide support and effectively perform their assigned job duties and contribute to the achievement of the program's goals and objectives of housing, income stability, healthcare goals, and assist with health homes tier transitions as needed.

  • Attending staff meetings, supervision and training as required.
  • Attend community meetings as requested and communicate with non-Step Up providers as appropriate and professionally representing Step Up.
  • Other duties as assigned and required for program compliance and success.

SKILLS: Leadership, motivation and management desires or experience; Strong assessment, interpersonal and intervention skills. Accuracy in charting of required documentation. Ability to train or guide staff to achieve confidence and ability to work independently in the field. Strong organizational skills and ability to delegate tasks to staff and monitor outcomes. Experience working with Homeless populations and field-based services.

Must have the ability to effectively communicate both verbally and in writing. Ability to work as a part of a team is crucial. Must be organized and detail oriented. Ability to present in a professional manner the ECM and ILOS-CS programs to community partners and advocate for SU members’ success in ending homelessness. Ability to work with disabled populations in a recovery-oriented context, embracing housing first, harm reduction and motivational interviewing techniques to ensure successful housing placement and end to homeless cycle. Demonstrate Step Up’s core values in day-to-day work.

QUALIFICATIONS: Bachelor's degree preferred or 6 years management experience. Experience working in case management is required. Strong computer skills with knowledge of Microsoft Office and intermediate MS Excel skills are required. Position requires use of own or company vehicle. Must have a clean driving record and current California’s driver’s license and insurance. Agency will reimburse for mileage.

SUPPLEMENTAL INFORMATION: Work with people experiencing homelessness whether on the streets, in shelters or other places of habitation or services, may present inherent challenges and difficulties such as: exposure to bed bugs or other infestations, unpleasant smells or odors, individuals who may have poor hygiene or unclean homes due to homelessness, mental health symptoms, or poverty. Additionally, in working with our members, employees may encounter instances of profanity, sexually explicit or derogatory language, or verbal or physical expressions of anger and trauma. Very rarely do these situations ever escalate to physical contact, and negative or derogatory communication patterns can often be negotiated successfully. All direct care staff are provided adequate training to develop skills to deescalate crisis situations that may arise and Step Up has established procedural safeguards for all employees to mitigate (but not eliminate) these inherent risks of employment.

PHYSICAL REQUIREMENTS: While performing the duties of this job, the employee is regularly required to sit; use hands and fingers; handle or feel; reach with hands and arms; talk; and hear. The employee is frequently required to walk, balance, stoop, kneel, and/or crouch. The employee must occasionally lift and/or move up to 15 pounds. Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and ability to adjust focus. Keyboard data entry required.
