Job Duties
- Implement training and support services to individuals as specified in Individual Support Plan (ISP). The areas of training and monitoring include, but are not limited to: housing, personal finance, mobility and accessibility, health and safety, relationships, recreation, self advocacy, job training, decision making and problem solving.
- Participate in the development and implementation of support plans
- Maintain all necessary individual records and evaluations
- Complete and maintain shift notes and medication documentation
- Participate in team meetings to discuss services
- Provide input for program planning
- Communicate as required/requested with team Lead and Facilitator concerning services, training, etc. with each individual you support
Qualifications include:
- A high school diploma or its equivalent
- A minimum of one year experience with adults who have developmental disabilities (or similar experience) is strongly preferred
- The ability to maintain First Aid & CPR Certification
- Position requires a valid California driver's license, a good driving record and a registered and insured vehicle
- Department of Justice level criminal background check
- Familiarity with SLS programs and the person receiving support is also beneficial
- Experience in de-escalation
Requirements:
- Personal Assistance: Ability to assist the individual with activities of daily living including bathing, toileting, and other self-help skills.
- Lifting / Physical Demands: Ability to lift 25 or more pounds frequently
- Challenging Behaviors: Ability and willingness to support individuals with challenging behavior
- Ability to read and interpret documents, write specific reports and correspondence, apply mathematical concepts to practical situations, solve complex problems, and to interpret written and verbal instructions is required
Shifts available
- Wednesday: 1pm - 5pm
- Thursday: 9pm - overnight - 9am
- Sunday: 8am - 2pm
- Sunday: 2pm - 9pm
